Below, you'll find other important information about this position. The MDS Coordinator takes a comprehensive assessment of each patient's needs and develops nursing care plans. Manages the MDS processes in accordance with government regulations and policies with the focus on maintaining excellent care for patients.
MINIMUM QUALIFICATIONS:
EDUCATION, CERTIFICATION, AND/OR LICENSURE:
1. Current unencumbered licensure with the WV Board of Registered Professional Nurses, or appropriate state board where services will be provided, as a registered professional nurse OR Current multi-state licensure through the enhanced Nurse Licensure Compact (eNLC).
2. Obtain certification in Basic Life Support within 30 days of hire date.
PREFERRED QUALIFICATIONS:
EDUCATION, CERTIFICATION, AND/OR LICENSURE:
1. Bachelors of Science in Nursing Degree (BSN) OR Associates of Science in Nursing Degree (ASN) or Diploma, currently enrolled in a BSN program and BSN completed within 5 years of hire.

1. Assists with the planning organization, development, and coordination of the care planning process upon admission of all new patients
2. Plays an active role in maintaining rapport with all departments to ensure a team effort is achieved in developing, implementing, and evaluating care plans for patients.
3. Assists in reviewing and evaluating the comprehensive assessment date and helps identify when and what additional assessments are triggered.
4. Completes and submits the MDS timely and accurately to ensure every MDS deadline is met.
5. Communicates effectively with residents and their families to ensure they are receiving excellent care and their needs are being met throughout their length of stay
6. Assists in maintaining the facility assessment calendar, coordinates the completion in of all MDS assessments, and ensures timely completion of assessments, care plans, and other required paperwork.
7. Checks and resolves MDS errors
8. Ensuring MDS and Care plan compliance with all departments.
