Job Summary:

The Coordinator is responsible for the assessment, intake, and service provision and planning for Bereavement Support Services. The Coordinator is responsible for overall program supervision and marketing to meet objectives for units of service and individuals served. The coordinator will be involved in case management and case consultations with other service providers in the Grey Bruce region. The coordinator will be responsible for the organization and implementation of bereavement and grief support groups, as well as 1:1 support of bereavement clients.

Responsibilities:

  • Oversight of Volunteer training, and performance coaching for applicable programs, facilitation of ongoing volunteer team meetings.
  • Monitors related program activities to ensure compliance with VON policies, procedures and standards for quality and client safety.
  • Achievement of annual targets for units of service and individuals served; tracking of program statistics.
  • Program marketing, including the development of a professional network relevant to the program objectives and direct to public marketing to support annual target achievement.
  • Development of strategic plan and quality improvement plan to ensure achievement of annual targets, and ensure quality client service delivery.
Client Intake and Assessment:
  • Review referrals and make contact with family or client where appropriate by the next working day.
  • Conduct the assessment and complete the required forms to initiate services.
  • Develop, implement, and regularly evaluate service delivery.
  • Make referrals to appropriate bereavement groups and/or match with a 1-1 volunteer.
  • Share relevant information with group facilitators and 1-1 volunteers.
  • Maintain up-to-date case notes in Procura
  • Follow up with clients as needed or as identified by the volunteer(s) and/or facilitators.
  • Make referrals to other services as required and requested.
  • Facilitate bereavement support groups when needed.
  • Provide education/advocacy to clients & families as needed.
Community Collaboration:
  • Attend meetings and events in the community that enhance VON Grey Bruce's profile, ability to strengthen services, and partnerships.
  • Build relationships with other individuals and agencies to provide strong referrals and case collaboration for more challenging situations.
  • Establish and maintain key relationships with doctors, mental health services, long-term-care centres, memorial committees, social workers, funeral homes, and others for the purpose of effective referrals and program development.
Risk Management:
  • Monitor risk management policies and procedures to ensure that program and organizational risks are minimized.
  • Evaluate the risks associated with each client and take appropriate action to control the risks.
  • Ensure the volunteers are able to work in a safe, healthy, and supportive environment in accordance with all appropriate legislation and regulations.
  • Ensure the clients have current information regarding their rights and our limitations.
Qualifications:
  • Post Secondary Education in Social Work, Health Services, Volunteer Management, Counselling, or other related field.
  • 3-5 years experience in a related field.
  • Knowledge of terminal illnesses, traumatic deaths, hospice/palliative care philosophy, grief and bereavement.
  • Demonstrated knowledge of client assessment and care plan development.
  • Knowledge and experience in gerontology is an asset.
  • Computer experience in Microsoft Office, Procura and Rai CHA assessments considered a strong asset.
  • Evidence of continued learning and ongoing education.
  • Experience in marketing, promotion, program development.
  • Demonstrated ability to network and build professional relationships and partnerships.
  • Excellent oral and written communication, interpersonal and organizational skills.
  • Ability to maintain confidentiality.
  • Ability to work flexible hours as needed to support volunteers and facilitate program marketing efforts.
  • Valid driver's license, insurance and use of a car is mandatory.
  • Willingness to travel throughout Grey and Bruce counties to client's homes.
  • Positive police check screening is a requirement of this position.
  • Bilingualism is considered an asset.
  • Must be able to wear Personal Protective Equipment (PPE)
Work conditions and physical demands: This role requires a detail-oriented approach in a dynamic environment, with physical activity including lifting, carrying (using proper techniques), bending, reaching, kneeling, and other movements that emphasize good body mechanics. Individuals in the role are required to walk, sit, stand, and climb stairs throughout the day, with some tasks requiring fine hand movements.
