Slate Healthcare is seeking a travel nurse RN Case Management for a travel nursing job in Gardner, Massachusetts.

Registered Nurse Care Coordinator, Case Management Department-
Schedule: Full-time

Requirements:
Education: BSN preferred.
Experience: Previous UR/QA experience required; 2 years of healthcare experience within Acute Care, SNF, HHA, Behavioral Health, and/or Insurance Industry preferred.
License and Certifications: Current Massachusetts Registered Nurse License.

Skills:
Interqual experience or equivalent preferred.
Proficient with Meditech Expanse.
Strong computer skills.
Excellent written, verbal, and interpersonal communication skills.
Exceptional critical thinking abilities.
Ability to multitask and flexibility essential.
Discharge planning experience, including care transitions, referral process, patient preference/choice services, patient & family satisfaction, post-discharge follow-up, etc.

Functional Demands:
Physical Requirements: Ability to exert 20 to 50 pounds of force occasionally, and/or up to 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects.
Other physical activities include reaching, handling, feeling, stooping, and kneeling.
Behavioral Attributes: Achievement motivation, flexibility, concern for order, initiative, self-confidence, self-control, customer service orientation, interpersonal effectiveness, teamwork, and information seeking.

Essential Functions:
Utilization Management: Conduct Utilization Review and Care Transitions & Coordination, including providing clinical information to payers, monitoring length of stay, seeking necessary care authorizations, and utilizing the InterQual Program.
Multidisciplinary Team Rounds: Participate in discharge planning rounds daily, working collaboratively with the team to determine each patient's needs.
Quality & Statistical Data: Review medical records for abnormal findings, complications, delays, and deviations from expected clinical outcomes. Acquire knowledge to keep up with changes in technology and regulations.
Denials/Appeal Process: Complete assessment of denial within 1 week, prepare written appeal letters, termination letters, discharge notices, and submit denials/appeals when completed to the department secretary for processing.
Discharge Planning: Develop, coordinate, and implement discharge plans, including identifying patient preference and selection choice for HHA/SNF placements.
