At UVA Continuum Home Health Care, you will use your skills as a physical therapist to provide care to clients in their homes. We offer direct clinical care PT jobs, providing high–level quality patient care in the community. As a home healthcare clinician, you are responsible for assessing, identifying, planning, and providing care with each patient in the home environment.
Our home health therapists work collaboratively to develop and implement a patient–centric plan of care for each client. Our PTs provide care to patients as part of regional interdisciplinary teams. As a home health physical therapist, you will work closely with community health providers, UVA Health, and others to ensure the safe delivery of care in the home. Our home care clinicians are challenged professionally, supported by a strong clinical leadership team and have a high degree of professional satisfaction.
UVA Continuum Home Health Care offers our therapy clinicians autonomy in their professional practice, provides scheduling support while also allowing self–scheduling and provides mileage reimbursement at the Federal level. We offer career advancement via our clinical ladder. We do not require candidates to have previous home care or OASIS experience. We provide an extensive onboarding orientation/training program for all new therapy clinicians who join our team.
UVA Continuum Home Health Care is the Home Health department of UVA Health. We are an academic home health agency, providing care in the community for almost 30 years. A Physical Therapist 2 consistently provides effective direct care to a variety of complex patients. In doing so the Physical Therapist 2 acts as a role model and supervisor for junior staff and helps identify patients who have unmet physical therapy needs. This level of therapist seeks and provides feedback for improved clinical practice and begins to assume a leadership role but seeks mentoring in this process. A Physical Therapist 2 is focused on expanding clinical knowledge and skills beyond the entry level and sharing this knowledge through in–services and mentoring PT1s in coordination of care with the other members of the interdisciplinary healthcare team. Therapists at this level may be asked to contribute to performance appraisals. The PT2 is expected to serve as a primary clinical instructor for students.
- Performs physical therapy examinations according to professional standards and departmental policy
- Demonstrates knowledge of intervention options and executes a program of skilled physical therapy intervention.
- Actively participates in professional growth of self and others.
- Actively participates in quality improvement and program development.
In addition to the above job responsibilities, other duties may be assigned.
MINIMUM REQUIREMENTS
Education: Bachelor's Degree in Physical Therapy
Experience: 6 months relevant experience
Licensure: Licensed to practice in Physical Therapy in the Commonwealth of Virginia. American Heart Association (AHA) Health Care Provider BLS certification required. Valid Driver's License required.
PHYSICAL DEMANDS
Job requires standing for prolonged periods and frequently bending/stooping/traveling. Proficient communicative, auditory and visual skills; Attention to detail and ability to write legibly; Ability to lift/push/pull 20 ? 50lbs. May be exposed to blood/body fluids and infectious disease.
