POSITION SUMMARY:
To provide a full range of occupational therapy services in a home care setting. Join an interdisciplinary team supporting children through Early Intervention and children up to age 18.
MINIMUM QUALIFICATION STANDARDS:
I. Education:
• Bachelor's degree in occupational therapy
II. Experience:
• One or more years of general OT experience with Bachelor's degree
III. License Requirements:
Vermont Occupational Therapy licensure
IV. General:
• Hold a valid driver's license and have a reliable, registered vehicle with insurance as required by the organization, and appropriate to position
• Broad base of clinical knowledge and skills in occupational therapy relevant to home care treatment
WORKING CONDITIONS:
1. Traveling is required
2. May work in community-based settings (childcare centers), and clients' homes and outdoors
PERFORMANCE REQUIREMENTS:
I. Performance Standards:
• Provide a full range of occupational therapy services to patients in their homes, including: ongoing assessments of current status; implementation of treatment plans, including specific therapeutic exercises/activities and instruction in using adaptive techniques and equipment in order to regain maximum independence, and the like; teaching patients and caregivers about their conditions and OT treatment plans, self-treatment and care.
• Confer regularly with supervisor, other clinical team members, departmental office staff, and other UVMHN Home Health & Hospice personnel to plan and coordinate activities, consult on cases, exchange information, investigate and resolve problems, and the like
• Other duties as assigned

• Reports concerns and complaints regarding abuse, neglect and exploitation in accordance with Vermont State Law and the organization
• Enhance or improve organization's services by participating in team meetings, projects and committees
• Contributes to a positive work environment and responds in a constructive manner to change
• Documents the provision care and services in accordance with organization's policy
