  • For Aya: Aya HIPAA Required
  • For Subs: Either signed Lotus HIPAA form (preferred) OR Core testing is acceptable.
  • Required - Primary Source Verification
  • Nursys verification required as applicable (required for all RNs)
  • Required as Applicable by Profession. Must meet or exceed the Angoff score or otherwise achieve a passing score if not done through Relias.
  • For QM - Audit form. DO NOT REQUIRE.
  • Initial physical, updated annually attesting to the ability to meet the physical demands of the offered position.
  • Negative PPD, Quantiferon Gold or T-spot required within 1 year of start. Updated annually.
  • Past Positive - clear Chest Xray within a year of relationship with Qualivis and annual TB Questionnaire.
  • 7 year criminal check within a year of start and updated annually.
  • Required within 30 days of start and updated for every contract renewal/extension following.
  • Proof of 2 clinical references (hospital eval also accepted)
  • If traveler has worked with us for over 1 year we must have an evaluation on file.
  • Complete Application/Resume
  • Qualivis: For applicable unit. Must be updated annually.
