About the role POSITION SUMMARY The Licensed Practical Nurse provides direct patient care, ongoing education and reinforcement of care plans, and providing general support for the patient under the physician"s direction. Includes preparing patients for exams, assisting during treatments, coordinating tests and follow-up care, and recording treatment in their electronic health record. Assists with explaining patient instructions to the patient/family. Demonstrates competency and skills to serve patients in geriatric populations. The Licensed Practical Nurse displays an elevated level of professionalism and engages resources and fellow team members within the center to deliver high level results. What you"ll do PRIMARY RESPONSIBILITIES
  • Assist the provider through data collection concerning the physical, psychological, social, and cultural dimensions of patients according to practice standards and policy/procedure.
  • Implements age-appropriate interventions based on individual patient's needs as directed by the provider.
  • Assist in evaluating the effectiveness of care given in progressing patients toward desired outcomes.
  • Provide ongoing education and reinforcement of care plans developed by the Provider based on identified health and education needs and the condition and age of the patient.
  • Demonstrates teamwork in the delivery of patient care.
  • Assist the Provider with triaging of patient portal messages.
  • Triage phone messages and provide feedback and answers to patient/physician/pharmacy calls.
  • Make calls to pharmacy, diagnostic facilities, etc. as required by the Provider.
  • Provision of basic wound care.
  • Perform suture/staple removal and dressing changes as indicated.
  • Medication reconciliation at point of care.
  • Preparation of prior prescription authorization requests for provider to review and finalize.
  • Manage/monitors providers clinical inbox in the EHR and triage as necessary per the direction of the provider.
  • Assist with preparation of clinical components for Provider review and finalization.
  • Preparation of medication refills for Provider review and finalization.
  • Assist in blood draws and IV insertions for fluid infusion under the supervision of the Provider.
  • Participates in center interdisciplinary team meetings related to patient care plans and outcomes of identified patients.
  • Ensures the overall quality of healthcare for each patient.
  • Ensures patient electronic health records are updated with the most recent medical treatments and records from outside facilities.
  • In addition to other job responsibilities, other duties may be assigned.
Qualifications REQUIRED QUALIFICATIONS
  • Graduated from an accredited Licensed Practical Nurse Program.
    Licensed to practice as a Practical Nurse (LPN) in the State of Maryland.
  • Minimum Experience: 5 years
  • Minimum Certifications and/or Licenses: CPR and first aid certification required.
  • Experience with EMR.
  • Phlebotomy (blood draw)
  • Professional demeanor and communication always.
  • Must be organized and attentive to detail.
  • Ability to manage competing priorities.
  • Resourcefulness in problem solving.
  • Able to take and follow through with assigned tasks and accountability.
  • Ability to work in a fast-paced environment.
  • Experience with Microsoft Office Word, Outlook, and Excel.
  • Participates in center and market interdisciplinary teams related to patient care plans, and outcomes.

The pay range for this role is:
22 - 27 USD per hour(Halethorpe (Baltimore))
