Schedule Details: Mon- Fri 8-430pm, weekend and holiday rotation

Our highly skilled team provides a wide array of services, including medication education and management, injections and intravenous therapy, wound care, nutritional counseling, bowel and bladder management and much more. Physical, occupational and speech therapists address home safety concerns, strengthening and rehabilitation needs. Our licensed social workers offer assistance with community resources, financial assistance and supportive care and counseling to patients and families.
What You Will Do:Works collaboratively with patients, nurses, social workers, physicians, other practitioners, caregivers and community agencies in coordinating and overseeing the interdisciplinary plan of care for a patient in a home care environment. Assesses plans, facilitates and evaluates timely coordination of quality, cost-effective care and works flexible hours and has 24-hour accountability as necessary to meet patient/family care needs.
What you will need:
- Bachelor's Degree in Physical Therapy or higher.
- State of WI Physical Therapist licensure.
- Valid State of WI Driver's License with acceptable driving record.
- Current BLS (CPR) certification or ability to obtain upon hire.
- At least 6 months experience in a home health setting preferred.
- At least 1 year physical therapy experience preferred.
