Schedule Details: This 1.0 FTE commits to 40 hours per week, every 3rd weekend rotation. Varying start time 6am - 1030am. May rotate to OMH, WMH, MUK.
What You Will Do:
Assists pharmacists, physicians and nurses with medication reconciliation processes to ensure that any differences between medications taken before admission and medications ordered upon admission for a patient are intentional and void of negative patient incident. Assists with reconciliation process at discharge in coordination with pharmacist and case manager. Offers discharge prescription service through ProHealth Retail Pharmacy. Collects appropriate insurance and demographic data for prescription processing.
Responsibilities:
- Adheres to the general hospital standards to promote a cooperative work environment by utilizing communication skills, interpersonal relationships and team building.
- Works cooperatively with others and organizes daily workflow to optimize output.
- Completes medication histories for new admissions.
- Promotes hospital's retail pharmacy program through offering of discharge prescription services.
- Provides service to customers to meet or exceed their needs and expectations.
- Collects and compiles data, statistics and other information needed for quality assurance programs or reports to provide information to assist with decision-making.
- Participates in the team approach to quality improvement to support the Pharmacy Department's commitment to the philosophy and principles of Total Quality Improvement.
- Maintains updated knowledge and skills and contributes to the education of others.
- Participates in planning processes by establishing personal goals to support quality improvement efforts and contribute to the achievement of departmental objectives.
- Provides age/developmentally appropriate care in accordance with Age/Developmental Specific Care Guidelines for the age groups served when participating in patient care.
Requirements:
- High School Diploma or equivalent
- Pharmacy Technician (CPhT)
- Registered with the State of Wisconsin
- Valid WI driver's license and in good standing
- Basic Life Support Certification (BLS)
