Summary:
Provide safe direct patient care and triage utilizing the nursing process in the ambulatory setting with a focus on patients with substance abuse disorder. Provide services and information to patients over the phone or virtually utilizing established protocols to determine the urgency of care needed. Facilitate intra/inter- and departmental collaboration and communication to address patient care initiatives. Utilize community outreach and resources to meet needs of patients. Provide patient education and direct care. Organize and facilitate the patient care team to streamline patient appointments and follow-up. Contribute to the functioning of the division as requested. Continuation of this position is contingent upon funding allocations.
Responsibilities:
* Communicate and work closely with staff and providers to coordinate processes involved in team-based patient care with a focus on patients with substance use disorder.
* Coordinate internal and external resources with the EMR to manage patient documentation and overall care
* Assist with the development of patient and workflow procedures with a focus on the provision of services for Medically Assisted Treatment (MAT).
* Coordinate and communicate patient care and documentation requirements with appropriate departments
* Provide direct patient care and education
* Assess patients' condition and nursing needs; set goals and utilize appropriate nursing interventions
* Assist with treatments and procedures including but not limited to Suboxone, Vivitrol or other medications
* Monitor patient progress during treatments including repeat inductions or other treatments
* Communicate any questionable or urgent symptoms to provider immediately
* Assist with or institute emergency measures for sudden, adverse developments in patients.
* Facilitate hand-off procedures with behavioral health and or other primary providers for continuity of care post treatment
* Collects data in conjunction with the quality improvement coordinators on treatment effectiveness to improve services provided
* Coordinate team meetings and events in collaboration with the Nursing staff
* Collaborate with division departments as needed for troubleshooting and problem solving
* Act as a resource for clinic personnel regarding substance abuse treatments and protocols as needed and/or requested
* Triage patient calls utilizing the nursing process and standard protocols
* Refer any emergent patient symptoms to immediate/urgent care per provider protocol
* Schedule appointments or direct callers to health care resources based on assessment and protocol suggestions
* Provide direct care to patients scheduled for Nurse Only appointments for the team
* Administer medications and perform routine treatments, ensure provider orders are carried out.
* Document all patient care and triaged calls according to established procedures
* Follow up abnormal laboratory or radiology results in conjunction with provider
* Ensure that orders and referrals are processed
* May be required to cover for nursing and clinic staff in other areas of the clinic when understaffed
* Assist infection preventionist with disease tracking and treatment.
* Participate in orientation and feedback of students and new personnel. May train or supervise students, as appropriate
* Identify and report/correct adverse environmental conditions
* Report potential or actual patient safety concerns, medical errors and/or near misses per protocols
* Ensure that equipment is maintained; monitors supply and medicine requisitions; maintains supply stock levels.

Essential:
* A valid driver's license or occupational driver's license, reliable transportation and insurance is required. Must obtain a Wisconsin driver's license or occupational driver's license within thirty (30) days of employment if applicant has an out-of-state driver's license. Must be authorized as eligible to operate a Personal and Tribal vehicle under the Oneida Nation's Vehicle Driver Certification and Fleet Management Law prior to actual start date. Must maintain driver's eligibility as a condition of employment.
