Description

At NorthBay Health, the Vice President of Population Health plays a vital role in advancing our mission to improve health outcomes, enhance care coordination, and optimize resource utilization across our health system. This position provides strategic leadership in care continuum services, utilization management, accreditation, patient safety, and quality improvement for both hospital and ambulatory settings. The VP is accountable for preventive care initiatives that improve patient outcomes, ensure compliance with Quality Incentive Programs, and oversee palliative and hospice services. By working across the entire care continuum, this role drives improved clinical and financial outcomes, guiding patients toward the highest quality care while supporting their long-term health and well-being.



Qualifications

  1. Education: Baccalaureate Degree in nursing preferred, or Baccalaureate Degree in a related healthcare field required. Master's degree in nursing or related healthcare field required. California RN licensure required. Certified Professional in Health Care Quality (CPHQ) preferred
  2. Experience: Extensive experience in healthcare leadership with a focus on population health, case management, utilization management, and quality required. Must have a strong background in data analytics, value-based care models, and strategic planning. A proven ability to engage physicians, staff, and community partners to drive outcomes is expected. Five years of leadership experience over Population Health, Quality and Care Continuum services strongly preferred.  Five years of demonstrated progressive management experience with increasing scope of responsibility with the last two years at Director level or above. Previous experience in leading Patient Safety with a focus on High Reliability and Zero Harm.  Previous experience working with Medical Staff Leadership.  Strong knowledge of accreditation and regulatory requirements (The Joint Commission, National Committee on Quality Assurance, California Department of Public Health, Centers for Medicare and Medicaid, AAAHC, IMQ, CMS). Must possess a thorough understanding of the quality payment program and Merit-Based Incentive Performance System (MIPS). 
  3. Skills: Knowledge of statistical tools and analysis. Demonstrated initiative, follow through and ability to work independently. Demonstrated results-oriented leadership style. Demonstrated effective project management skills. Excellent interpersonal, judgment and problem-solving skills Excellent written and verbal communication and presentation skills. Knowledge of and experience with organizational change processes. Must be able to deal with job-related stress, multiple projects and deadlines.  Basic computer skills.  EHR experience.

  5. Compensation: $280 to $340 based on years of experience doing the duties of the role.
