Description

At NorthBay Health the Hospice – Associate Case Manager Nurse has the responsibility, accountability, and the authority to deliver safe, effective, quality, excellent nursing care according to the philosophy and goals/objectives of the Department of Nursing.  The RN, utilizing the Jean Watson's Theory of Caring, the Relationship Based Care, and the NorthBay's professional nursing practice model assesses, plans, implements and evaluates care for all patients assigned to his/her care. The RN makes determinations regarding the progress of the patient toward goals and utilizes a therapeutic professional relationship with the patient/family and interdisciplinary team to facilitate the promotion, maintenance, and restoration of health.  Care is provided according to the specific physiologic, developmental and psychosocial needs of the patient.  Supports the mission, vision, values and expectation of excellence of the organization. 

Works under the supervision of the Hospice Manager. Provides quality skilled nursing care to patients in the home setting per physician's orders, complying with guidelines, standards and regulations. Coordinates patient care with other disciplines and case manager.  May be asked to carry a case-load in response to an increase in the census



Qualifications

Education:

  • Graduate from an accredited school of professional registered nursing.
  • Minimal nursing degree of BSN level required.  

Licensure/Certifications

  • Current RN license by the State of California.
  • Current AHA or equivalent BLS.
  • Current national certification in nursing specialty in Hospice is preferred.
  • Hospice certification is CHPN (Certified Hospice Palliative Nurse) preferred.

Experience:

  • One year of work related experience as a registered nurse in Hospice is preferred.

Skills:

  • Proficient written, verbal, and electronic communication skills.

Compensation:

  • Hourly Salary Range MIN $69.81 –  MAX $84.15 (Offered hourly rate based on years of experience)
  • 15% per diem differential available for qualified staff
