Description
At NorthBay Health the Hospice Home Health Aide (HHA) is responsible for the delivery of patient care to assigned patients under the direction and supervision of a licensed, professional nurse or licensed therapist. The Hospice HHA, utilizing the Jean Watson Nursing Theory of Caring and the Relationship Based Care Model, provides assistance to patients in accordance with the functional and developmental needs of each patient. Supports the mission, vision, values and expectation of excellence of the organization.
Duties may be adjusted at the direction of the case manager or clinical supervisor based on the evaluation of the patient's needs.

Qualifications
- Education: High school graduate or equivalent preferred.
- Licensure/Certification: Graduate of certified program for HHA, and certification by the State Department of Public Health, with a current California HHA certificate. Current AHA or equivalent CPR certification. Maintains continuing education units. Current California driver's license with automobile insurance per agency guidelines.
- Experience: One year of experience as Home Health Aide in the past five years. Must demonstrate and maintain current knowledge and skill in providing appropriate care for patients in the Geriatric, Adult age groups.
- Skills: High level of initiative and independent work skills. Excellent organizational and time management skills. Able to understand oral and written instructions in English. Must demonstrate and maintain current knowledge and skill in providing appropriate care for patients in the following age groups: Geriatric, Adult.

- Compensation: Per Diem hourly rate $27.82 - $30.71 (based on years of experience)
