Patient Diagnoses- inpatient and outpatient neonate to 26 years of age patients
Special Procedures/Unit Details- Neuro, Body, MSK, Angio, Spect., sedated cases and non-sedates.
Special Equipment - GE and Siemens 1.5T and 3T scanners, Bayer Injectors, EPIC, Invivo Monitoring, GE PACS

2 Manager or Supervisor references required. Must be a professional reference. Charge RN is acceptable for one of the references.
Peer references are not acceptable.
Unit specific skills checklist required. Update annually.

SUBS: Attestation required
Current government issued photo ID is required within 24-48 hours of signing contract. Temp DL ok to start, but must have perm DL on file once available.

If traveler is undergoing a name change, proof of name change is required for approval.
Legal name must be reflected in NOVA/Connect
