• Position/Specialty: Nurse Case Manager
• Ratios: 1:30
• Shift: Will work 5-8 hour days, 8am-4:30pm with holidays as needed
• Weekend REQ: 2 weekend shifts per contract
• Is on-call REQ? No
• Will clinician float within scope to meet facility needs? No
• Looking for a Nurse Case Manager (NCM) I reports to the Manager of Case Management and Care Transitions.
• Under guidance of the Nurse Case Manager Team Leader, the Nurse Case Manager I functions as a member of the clinical team.
• This role coordinates a plan of care to achieve clinical outcomes and promotes a safe and timely discharge.
• This position manages the plan of care throughout the care continuum to ensure the efficient movement of the patient through the hospital system.
• The Nurse Case Manager I serves as a clinical expert related to discharge planning, resource utilization, and case (sensitive information removed).

Job Requirements
• Years of experience REQ: 3 years of RN experience
• First-timers accepted: No- Must have Travel Experience
• Education REQ: BSN
• Experience Preferred: Prior leadership experience and Epic Experience

Schedule Information
• Shift: Will work 5-8 hour days, 8am-4:30pm with holidays as needed
• Weekend REQ: 2 weekend shifts per contract
• Is on-call REQ? No
• Open to accommodating block schedule? Approval prior to assignment, but not guaranteed.
• RTO Restrictions: All RTO must be included at time of submission. Approval will be based on facility's needs.
• Guaranteed Hours: Facility allowed to call off 1 shift per 2 weeks
