Mercy is currently seeking a board-certified physician with acute inpatient care experience to lead our exciting Hospital-at-Home program in St. Louis. Missouri, where Mercy patients get their hospital care at home.

 

Hospital care-at-Home is a unique opportunity to be a “guest” in a patient's home, serve your community, and transform the care experience.

 

Patients who have presented to the Emergency Department and have acute care needs that require hospitalization now have the option to be treated in the comfort of their home by our dedicated and highly skilled team. They are thoroughly screened and admitted in the Mercy Hospital care at home Program. Once “admitted”, these patients are transported home to receive all the acute care services during the typical 3–5-day admission. This includes twice daily nursing visits, daily physician or advanced practice provider visit, 24/7 medical direction oversight, medication management, equipment, and all additional program support during HaH episode of care.

 

This Position offers:

  • Opportunity to join growing team as a leader over-seeing the clinical, quality an safety components of inpatients in the home and virtual environment.
  • Role will be divided between clinical patient care and leadership and driving growth of the program.
  • Continued opportunity for further leadership potential as program expands across the ministry
  • Will lead a multidisciplinary care team  in the identification and treatment of patients that meet criteria for admission to the home.
  • Provide clinical care to acutely ill patients on our service by providing either virtual or in person visits daily depending on patient needs and circumstances including treatment plans, medication management, and medical direction oversight.
  • Perform a daily patient care visit and all required clinical documentation, orders, chart review to meet the HaH policies and procedures.
  • Facilitate in-home diagnostics, point-of-care testing, administration of medications and other treatments, patient monitoring, and any other patient needs as they arise
  • Participate with quality measurement and/or quality improvement initiatives to advance the work of HaH and other alternative care pathways, including advancements or innovations of the care model
