Overview:

 

The Patient Educator-Diabetes is responsible to use the Nursing Process to meet the needs of patients according to the profession standards of nursing care. Assures that the philosophy, mission, goals, objectives, policies and procedures of the Nursing Department and Mercy Hospital are followed. The Patient Educator-Diabetes is responsible for the quality of care of a caseload of patients; delegates appropriately and directs staff in the delivery of care; assures responsibility for the appropriate utilization of resources; collaborates with medical staff and others in the provision of care; demonstrates positive interpersonal relationships with others in communication and problem resolution; is sensitive to the needs of patients, families, and fellow coworkers. The Diabetes Educator plans and provides for patients in accordance with age-specific needs; plans and provides for patients in accordance with any identified cultural diversity needs (such as religions, lifestyles, beliefs, physical capabilities, and cultures); plans and provides for patients by assuring appropriate and effective pain management; assess, report, and assist any patients that present with evidence of abuse and/or neglect; and supports the mission of Mercy. Co-workers are expected to abide by Mercy Hospital South safety and infection control policies to maintain a safe environment for patients and co-workers. Supervises. None NOTE. Competent to provide education to the age of the specific population of assigned areas. See 'Age-Appropriate Care for Neonate, Pediatric, Adolescent, Adult and Geriatric Patients' policy. Current CPR Certification is a required element to maintain the job position.

 


Qualifications:

  • Education: Graduation from an approved school of nursing. . 
  • Licensure: Current RN license with the Board of Nursing in the applicable State of practice.
  • Experience: Must be a registered nurse with a minimum of 5-7 years clinical experience. Knowledge, skills and experience with adult learners is needed. Must be able to implement and revise teaching outline using objectives and activities that will enhance patient/family understanding of patient's condition. Strong human relations skills. 
  • Certification/Registration: Must have American Heart Association Healthcare Provider card or complete a course within probation period. TNCC, ACLS and ENPC or PALS.Skills, knowledge and abilities: Demonstrates skills in human relations and communication, written verbal, and electronic. Demonstrates clinical competence in patient care delivery. Possesses the ability to assess data and assist personnel in providing appropriate level of care. Performs responsibilities while fulfilling the Philosophy of skills, knowledge and abilities.
  • PATIENT CARE:Provides education to the patient with Diabetes Mellitus according to the American Diabetes Association guidelines.- Provides education to patients of all ages in a personalized and compassionate manner utilizing the Nursing Process.Assists with discharge planning and case management.
    • DOCUMENTATION:Demonstrates compliance with the standards of nursing documentation.- Documents in a manner which is complete, accurate and legible.- Completes chart forms appropriately.
    • EDUCATION:Implements educational programs for patient, family, students and staff.- Identifies educational needs of patients and documents same as related to over-all plan of care.- Completes teaching as assigned and documents patient and family level of comprehension.
    • CONTINUOUS QUALITY IMPROVEMENT:Demonstrates commitment to quality patient care.- Actively participates in the Quality Improvement program as indicated and identifies concerns where improvement will enhance patient care.
    • PLANNING/RESOURCE MANAGEMENT:Sets priorities and utilizes time efficiently in order to complete work on time. Insures items are properly charged.
    • COMMUNICATION:Maintains effective communication through appropriate utilization of the chain of communication.-Communicates verbally and non-verbally in a concise, effective and appropriate manner.Displays a positive attitude and is adaptable to change.
    • PROFESSIONALISM/GROWTH:Participates in programs and projects directed toward professional growth and development to meet requirements for American Diabetes Association.- Attends and contributes to meetings, utilizes information and shares with others.Performs other duties as assigned or requested.
