Description

The Nursing Care Manager is responsible for guiding the field training experience for newly hired clinicians and mentoring them throughout the first year of employment to enhance engagement, retention, and clinical skill development. This role involves providing clinical support, training guidance to field staff, and ensuring high-quality decision-making and care delivery. The Nursing Care Manager may perform patient care and case management duties as needed in the field, collaborating closely with the clinical training specialist and Clinical Operations Manager. Additional responsibilities include contributing to strategic goals, covering for the Clinical Operations Managers or Professional Development Specialist, supporting quality care, and managing aspects of clinical workflow, including case conferencing and staff coordination. In partnership with education, quality, and operations, the Nursing Care Manager develops, and updates individualized clinical orientation programs for new hires, facilitates the transition from orientation to entire team integration, and serves as a classroom and field trainer to support best practices. The Nursing Care Manager plays an active role in quality assurance (QA), offering training based on QA findings, conducting joint visits, chart audits, and participating in mock surveys. This role supports operations and quality in complaint and occurrence resolution. The ideal candidate is a knowledgeable and collaborative clinical resource that fosters a supported and skilled clinical team.
Education

  • Bachelor's degree in Nursing from accredited program or combination of education with 3 years relevant nursing experience including 3 years experience in a certified home care agency or hospice; or promotion from MHHC Clinical Lead position. required
Experience
  • 3-4 years Relevant nursing experience with Associate's degree or 2 years relevant nursing experience with Bachelor's degree required and
  • 3-4 years experience in a certified home care or hospice program required and
  • Prior preceptor experience preferred
Licenses and Certifications
  • RN - Registered Nurse - State Licensure and/or Compact State Licensure Valid RN license in all jurisdictions in which the employee provides services to patients, Maryland, DC or Virginia or any combination based on work location(s) Upon Hire required and
  • DL - Valid State Driver's License in good standing with a dependable vehicle for transportation Upon Hire required and
  • CPR - Cardiac Pulmonary Resuscitation (includes BLS and NRP) for healthcare providers from either the American Heart Association (AHA) or American Red Cross within 90 Days required and
  • Additional unit/specialty certifications may vary by department or business unit.
Knowledge, Skills, and Abilities
  • Demonstrated ability to teach, direct and guide professionals to gain desired results. Demonstrated ability to function at a high level of independence and autonomy. Strong leadership and delegation skills required. The ideal candidate will have a passion for teaching and excellent interpersonal skills, including verbal and written communication. IV therapy, Foley catheter, venipuncture, wound care/vac, and comprehensive medical-surgical skills. Organizational skills. Excellent verbal and written communication skills. Basic computer skills required. Knowledge of home equipment/DME, home care reimbursement, and regulations governing LPN's and HHA's.
  • Contributes to the achievement of established goals and objectives established from MedStar management team. Adheres to department policies, procedures, quality standards, and safety standards. Completes annual Code of Conduct, Compliance and other mandatory training. Complies with governmental and accreditation regulations.
  • Serves as lead preceptor responsible for overseeing the field training experience for newly hired nurses and home health aides. Collaborates with and guides field trainers/preceptors to provide new hire's the learning experiences they need to meet clinical competency requirements.
  • Acts as coverage to support operations in the absence of the Clinical Operations Manager. Participates in interviewing, hiring, and retention of clinical staff.
  • Responsible for specific portions of administrative workflow related to case conferencing and coordination, quality care, and matters related to care delivery.
  • Collaborates with the clinical training specialist and Clinical Operations Manager to support an individualized clinical orientation program for new staff. Updates the program as needed to meet learning needs.
  • Serves as field educator in the discipline, providing feedback and suggestions to implement best practices and standardize procedures.
  • Maintains a caseload of patients and meeting the expectation of care described in the corresponding clinical job description. Performs admissions, revisits, recerts, discharge and joint visits to ensure quality patient care delivery and excellence in patient experience.
  • Works in collaboration both remotely and in-office as applicable on a routine basis with the Clinical Operations Manager and Operations Support Specialists.
  • Acts as field mentor to clinical staff and provides Clinical Operations Manager and/or RN clinical training specialist with input regarding team members' work performance. Recommends and provides additional education based on clinical skill needs or problems identified through the QA process or at the request of the staff member or Clinical Operations Manager and/or RN clinical training specialist.
  • Conducts chart reviews during newly hired associates' first year and assures that admissions, recerts, and discharges in the patient documentation are accurate and that proper utilization and clinical needs are being achieved.
  • Performs Joint Visits and participates in mock surveys as directed.
  • Partners with Quality and Clinical Operations Manager to assist with patient complaints and occurrence resolution. Communicates with patients, caregivers, and interdisciplinary staff to resolve actual or potential safety issues with the patient.
  • Maintains effective working relationships with other departments. Participates in multidisciplinary quality and service improvement teams, meetings and on committees representing the department and agency in community outreach efforts. Enhances growth and development and enriches personal knowledge and skill through participation in educational programs and affiliations.
  • Recommends and provides additional training based on clinical skill needs or problems identified through the QA process or at the request of the staff member or Clinical Operations Manager and/or Education Department/Director.
  • Serves as clinical resource/triage nurse to operations team, patients, caregivers, and physicians. Assists in the clinical distribution and proper care of patients by supporting field staff in decision-making, case conferencing, and management of schedules.
  • Demonstrates advanced knowledge base of Oasis documentation/accuracy. Ensures compliance of documentation regulatory requirements including: care plan building, goal writing, and guiding clinicians to proficient documentation.
  • Participates in meetings and on committees and represents the department and hospital in community outreach efforts. Participates in multidisciplinary quality and service improvement teams.
