Description

Main office is at MedStar Harbor Hospital. Travel is required to all 4 Baltimore hospitals and 8 community health sites that the Mobile Health Clinic is stationed. Occasional evening and weekend hours during events required.

The Mobile Health Clinic Practice Manager is responsible for overseeing the day-to-day operations, staff supervision, administration, community engagement, grant management, and performance data for the MedStar Health Baltimore Region's Mobile Health Centers. Under the guidance of the Regional Director of Community Health, this role ensures smooth operations, develops/implements administrative protocols, supports patient care services, manages budgets, grant reporting, and fosters positive team dynamics. The Practice Manager will drive efforts to enhance patient engagement and fulfill our department mission of bridging gaps in health care by improving holistic health outcomes. The Practice Manager also manages community outreach events and coordination with community partners. The ideal candidate will have experience with outpatient practice management for underserved communities and be adaptable to a fast-paced environment.


Education
  • High School Diploma or GED required or
  • Bachelor's degree in Health Care Administration, Business, Public Health or other related field required and
  • Courses in medical terminology preferred
Experience
  • 3-4 years healthcare/medical office experience required and
  • 2 years general practice management experience; preferably in an ambulatory care setting with underserved populations required
  • 1-2 years Project management and/or managing population health initiatives. Experience in finance, data analysis, and community outreach preferred
  • One year of relevant professional-level work experience may be substituted for one year of required education.
Licenses and Certifications
  • Certified Medical Office Manager, Medical Assistant, Registered Nurse preferred

  • Contributes to the achievement of established department goals and objectives and adheres to department policies, procedures, quality standards, and safety standards. Complies with governmental and accreditation regulations.
  • Provides oversight and management for the daily operations of the medical office and ensures patient access to care including scheduling, staff coverage and workflow management. Responsible for creating and maintaining a professional environment in the medical office. Adjusts operations as needed to maintain peak performance. Establish and maintains appropriate levels of equipment, office and medical supplies to support the medical practice.
  • Responsible for maintaining positive, productive, and professional relationships within the practice, as well as between the providers and the health care system. Prepares the mobile health clinic for the addition and departure of providers; ensures smooth operational transitions. Assists with growth and development of the mobile health center by working closely with senior leadership. Coordinates and leads meetings with the providers and staff on a regular basis to keep team informed of MHS and MHC goals, practice goals and policy changes. Prepare and deliver presentations to department leadership for quarterly program updates.
  • Coordinates the schedule for new providers and modifies existing provider schedules as appropriate. Assists with hiring, onboarding, and orientation for clinical staff in collaboration with Clinical Lead. Monitors provider scheduling practices and staffing levels to ensure patient needs and financial goals are met. Monitors provider schedules on a daily basis to maximize appointment availability and ensure same-day access is available. Manages the IDX scheduling system-input schedules, troubleshooting the system, and helping others using the IDX system.
  • Ensures patient satisfaction with the medical group's performance on all levels. Responds timely to patient concerns and requests for information. Engages in service recovery activities as appropriate. Routinely reviews CGCAHPS data and develops action plans to address areas for improvement.
  • Monitors and ensures accuracy of recording on patient medical records. Maintains all required reports, records, statistics, etc. Prepares and maintains records of patient charges.
  • Monitors medical condition of patients and reports changes to appropriate personnel. Monitors emotional stability of patients and reports changes to physician or charge nurse. Performs basic nursing procedures including thermal applications, nonsterile soaks, application of simple dressing, perennial care, and running tests.
  • Performs clinical procedures within scope of practice after demonstrating proficiency in procedural skills pertinent to practice/specialty area. Prepares patients for scheduled procedures to assist in patient assessment.
  • Provides direct patient care, evaluates outcomes, consults with other specialists as required, and adjusts nursing care processes as indicated to endure optimal patient care.
  • Provides preventive healthcare services to long-term patients and arranges consultations and referrals.
  • Maintains and coordinates all staff schedules to accommodate patient flow. Responsible for providing backup to key positions during peak times and staff absences. Based on skills, this may include direct patient care services. Performs weekly payroll activities for assigned staff. Actively engages in recruiting, hiring, orienting, promoting, reviewing and terminating staff according to organizational policies. Maintains up-to-date staff files of required certifications and licenses. Fosters staff engagement and productivity.
  • Develops, executes and monitors Action Plans related to Associate Engagement Surveys. Collaborating with the clinical leads on implementing and managing new technologies, systems, and quality improvement initiatives to ensure staff and patient satisfaction. Travels between mobile health program sites, the MedStar Health Baltimore Region hospitals, and other locations as needed for operations management, event coordination, and other functions related to the programs.
  • Assists with grant application writing processes and continual monthly narrative and data reporting as it relates to the community health needs assessments and goals of the Mobile Health Center Programs. Coordinates and manages outreach efforts, engages with internal and community partners, and participates in relevant community external events at the request of the community. Represent the department at meetings, committees, and community outreach events (occasional evening or weekend hours), advocating for program success and supporting community health goals.
  • MHCP locations: Handles urgent exacerbations of chronic illness, new urgencies, and come medical emergencies, including same day or next day visits for urgent events and telephone communication with the responsible physicians. Determines whether patients can stay at home or need hospital care and coordinates same day services. Develops a plan to handle the urgent patient issues and modifies the overall goal of care. Identifies situations requiring emergent care and coordinates patient transportation to hospital.
  • MHCP locations: Provides continuity of care that is appropriate for every stage of illness, including at the end of life. Creates goals of care that include individualized and evidence-based care plans to guide patients, their family members, and care givers through the projected trajectory of illness. Manages patients' transition from hospital back to home after acute care admissions. Partners with social work to determine necessary at home care assistance from family members and care givers. Provides ongoing and problem-oriented care to frail elders with multiple chronic illnesses by making house calls.
  • Anesthesiology locations: Conducts preoperative/pre-anesthesia patient review of systems and/or history and physical on any patient undergoing anesthesia. Clinical functions are performed under and in collaboration with the general medical directions of MedStar Medical Group Anesthesia and the specific medical director of Pre-Anesthesia Testing. Orders, reviews, and communicates findings appropriately of required laboratory tests, diagnostics tests, and physical assessment. Conducts appropriate patient education activities and interventions related to perioperative care including but not limited to the management of diabetes, anemia, and frailty.
  • Participates in meetings and on committees and represents the department and hospital in community outreach efforts. Participates in multi-disciplinary quality and service improvement teams.
