Job Summary: 

MedStar Health is looking for a Lay Patient Navigator to join our team at MedStar Health Good Samaritan Hospital. The ideal candidate will have excellent organization skills, attentiveness to detail, compassion, communication, and the ability to navigate through complex health systems. 

The Lay Patient Navigator will serve as a guide helping patients traverse the healthcare system, ensuring their needs are met, overcoming obstacles limiting the patient's ability to receive appropriate care and treatment. This will include arranging follow-up physician and ancillary services for both Emergency Department and hospitalized patients. The navigator will be a liaison between patients, hospital staff, and community physicians. The navigator will gain expertise on community resources available to the patient and families, working in collaboration with social workers and case managers to ensure optimal patient care.

Primary Duties: 

  • Assists in coordination of follow-up medical evaluations and ancillary services, ensuring continuation of care and connection to community resources promptly.  

  • Promoting patient satisfaction and quality of care by facilitating communication between the patient, family, and healthcare providers.  

  • Informing the patient on financial implications of care in association with insurance and providing patient with appropriate resources as needed.  

  • Maintains an accurate database of medical and ancillary service referrals and percentages of completed follow-up appointments. Additional data collection, as necessary. Markets the services of the Patient Care Navigator program. Participates in meetings and on committees and represents the department and hospital in community outreach efforts 

  • Assists patients when appropriate to outpatient medical centers once medically cleared with a Medical Screening Exam in the Emergency Department or when ready for discharge from IP units.  

 

Qualifications: 

  • High School Diploma or GED. 

  • 5-7 years Related healthcare or customer service experience.  

  • 5 years of postgraduate social work experience. 

  • Bachelor's degree preferred. 

 

This position has a hiring range of $20.17 - $33.21
