Description

Leads, manages and supports pharmacy personnel in ensuring optimal pharmaceutical care with a specific emphasis on continuous quality improvement, performance enhancement, cost-effective formulary management and human resource management, and regulatory and accreditation compliance across the entire spectrum of medication use processes throughout the organization. These functions are performed in accordance with all applicable laws and regulations and National Rehabilitation Hospital's philosophy, policies, procedures and standards.
Education

  • Bachelor's degree from an accredited college of pharmacy required
  • Doctoral degree from an accredited college of pharmacy (PharmD) preferred
  • Completion of an ASHP-accredited residency or fellowship (PGY-2 leadership residency) preferred
Experience
  • 3-4 years experience in a hospital or health system (to include management or supervisory experience)
  • Demonstrated experience with JCAHO accreditation standards and surveys, Federal and state laws and regulations, and performance improvement initiatives, measurement and benchmarking techniques.
Licenses and Certifications
  • PHARMD - Licensed Pharmacist in the District of Columbia
  • Understands and maintains environment of care through successful completion of annual re-certification for OSHA, fire, disaster, blood borne pathogens or other pertinent competencies.

  • Manages the implementation and actively leads in accomplishing the pharmacy service's scope of practice.
  • Coordinates and manages the implementation of the organizational and Departmental performance improvement plan. Manages the development, design, implementation, and monitoring of the performance improvement plan. Seeks input from pharmacy personnel for revisions to the performance improvement plan.
  • Manages and oversees the collection, tracking, trending and review of the data to measure performance improvement progress (including MUE and ADE reports) and assuring that such data is disseminated to the health care team and, when appropriate, organizational committees.
  • Coordinates the tracking, trending and reporting of pharmacist's interventions to ensure continuous quality improvement and to assess pharmacists' performance.
  • Evaluates and analyzes adverse drug event (ADE) data, identifies opportunities for systemic improvement, and works collaboratively with the health care team to improve system processes.
  • Manages the collection, tracking, trending, review and reporting of medication variance data, conducts appropriate root cause analyses of medication variances and develops suggestions for the future prevention. Disseminates information and data to pharmacy personnel and solicits feedback for performance improvement initiatives for medication use processes.
  • Manages and leads risk reduction initiatives affecting medication use processes.
  • Actively participates on and/or facilitates intra- and inter-departmental committees that address performance improvement initiatives focusing on medication use processes
  • Coordinates and manages the medication use process for pharmacy services. Assures timely, effective dissemination of medication use related information to pharmacy personnel and other health care professionals. Coordinates the tracking, trending and reporting of pertinent information to pharmacy personnel related to medication use processes.
