As a Care Coordinator you will be responsible for coordinating patient support functions for MedStar Total Elder Care patients, families, and caregivers. Provides daily operational guidance and coordination to an interdisciplinary team and monitors the complex, high volume of operations associated with home-based primary care. Key team member who supports the interdisciplinary team while contributing to the team's success and ensuring that patients and families are treated courteously and respectfully.

Primary Duties:
Completes preliminary registration for new patients and updates information on existing patients in scheduling system. Obtains necessary demographic and insurance information, referrals, pre-certifications, signatures, copies, etc. to ensure timely appointments. Troubleshoots and resolves any issue preventing patient appointments.
Contributes to the achievement of established department goals and objectives and adheres to department policies, procedures, quality standards and safety standards. Complies with governmental and accreditation regulations.
Demonstrates behavior consistent with MedStar Health mission, vision, goals, objectives and patient care philosophy.
Develops and delivers weekly new MedStar Total Elder Care member orientation. Designs and participates in outreach events.
Maintains MTEC team calendar of activities and meetings, clinician locations, on-call schedules, and patient appointments.
Organizes and monitors the daily workflow for all patient support functions to accommodate a high volume of activities and rapid turnaround requests from Geriatricians, Nurse Practitioners, Social Workers, patients and families/caregivers.
Organizes and participates weekly team meetings on MTEC patients, attended by clinicians, to assure needed coordination between clinical and patient support functions.
Participates in multidisciplinary quality and service improvement teams as appropriate. Participates in meetings, serves on committees and represents the department and hospital/facility in community outreach efforts as appropriate.
Performs other duties as assigned.
Receives incoming calls and messages and appropriately addresses or triages the request by providing information about MTEC, scheduling appointments, completing documentation, submitting prescription refills, or taking messages.
Schedules appointments for patients, including coordinating appointments with other medical offices for tests, including pre-admission testing. Confirms appointments; may give patient instructions explaining upcoming procedure/test and/or answer questions. Arrange transportation, follow up on appointments.
Serves as the liaison between MedStar Total Elder Care and various community services (home health agencies, pharmacies, DME companies) to assure follow-up of clinician orders for patients.
Sorts and distributes incoming mail and faxes to include scanning and indexing appropriately.
Utilizes Electronic Health Record systems to create and maintain patient medical records.
Qualifications:
High School Diploma or GED.
Associate degree preferred.
2 years' experience working in a medical office and/or a service business required.
Experience working with geriatric patients preferred.
This position has a hiring range of $20.17 - $35.04
