Description

The New Grad Nurse (< 12 months of experience) is responsible for providing and documenting skilled nursing care in accordance with care developed for each individual patient in accordance with physician's orders when applicable.

Essential Duties and Responsibilities:

1. Utilizes the nursing process to plan, implement, and evaluate patient/client care and/or

contracted services

2. Demonstrates on each assignment the provision of skilled nursing care rendered in accordance with prescribed orders

3. Identifies and documents changes in patient's both physiologic and psychosocial condition

4. Notifies Director/Supervisor if applicable on the following;

a. Physician notification to obtain necessary orders for intervention(s) per state

regulations

b. Changes in patient/client condition

5. Adheres to Maxim's policies and procedures and applicable protocols

6. Identifies and prioritizes actions based upon patient/client care requirements

7. Assures patient safety at all times and recipient of services provided

8. Verifies that documentation reflects patient/client collaboration, participation in plan of care, expected patient/client outcomes, and selected nursing interventions

9. Performs interventions, treatments, and medication administration, according to identified priorities, plan of care, facility policies and patient/client care outcome standards as applicable.

10. Monitors and documents effectiveness of interventions, treatments, and medications

11. Evaluates patient/client and provides a safe, comfortable and therapeutic patient/client

environment

12. Monitors the safety and effectiveness of the environment and equipment including universal precautions

13. Participates in activities to facilitate good interpersonal communications

14. Communicates observations and significant changes in patient/client status to the Director of Clinical Services and/or Clinical Supervisor /Case Manager or licensed designee

15. Documents all physician collaboration/coordination of care in the medical record and

16. communicates this information as needed

17. Attends case conferences, team meetings, education in-services as required

18. Supports professional practice by observing the following;

a. Nurse Practice Act

b. Standards of Nursing Practice

c. Standards of Patient Care

d. Standards of Accrediting Body (ACHC, Joint Commission, etc.)

e. State and Local Laws

f. Patient Bill of Rights

g. Maxim Healthcare, Nursing, and Client's Hospital Policies

19. Performs other duties as assigned

Minimum Requirements:

1. Graduate of an accredited school of nursing

2. License Required

3. 3.0 GPA preferred

4. Complies with accepted professional standards and practice

5. Current CPR

6. PPD or chest X ray if applicable

7. Successful completion of Maxim Homecare Novice Nurse Program orientation and training

Compliance & Ethics Expectations:

- Participates and successfully completes the company's compliance program requirements and adheres to the Code of Conduct, Company policies, and applicable federal and state requirements

- Sets an example for other employees regarding how the Company's Code of Conduct and Compliance Program is applied and observed every day when dealing with customers, business operations, or other team members

- Reports potential violations of company policy, Code of Conduct, and/or applicable laws and regulations to the company hotline, through the chain of command, to the Compliance and Ethics Department, or through other channels made available by the company for reporting potential violations

- Promotes an environment in which other employees are encouraged to report potential violations

- As appropriate, provides input and suggestions regarding areas in which policies, procedures, workflows, and/or controls can be improved to enhance compliance
