This is a multi-function role applying concepts from a care delivery system that values a primary nurse model. - Paired with a medical oncology care team, the Practice Nurse plans and advocates for that team's new and established patients. - The nurse coordinates many functions within the department, as well as providing guidance to ancillary staff during day-to-day operations. Specific to this role, order entry, triage, coordination of all new referrals, established and sick visits within provider practice. This role encompasses the clinical communications, time management and critical thinking skills unique to the experienced oncology RN. As such, the primary goal of this team member is to provide compassionate, quality care within a seamless delivery system to all patients within the medical oncology department based on patients' physiological, psychosocial, and spiritual needs. Primary Responsibilities: Begins formulating a working plan of care with the care team, applying initial assessment, patient data and experiential knowledge. Active participation in multidisciplinary team conferences. Perform chemotherapy teachings (inclusive of supportive care) and reinforces established plan of care. If patient has had teaching at another organization (outside of MGH), the Practice Nurse arranges for a teaching session according to standards at the Cancer Center. Thorough and accurate documentation of all patient information and patient encounters in the EMR. Handle timely triage of patient messaging (e.g.: Patient Gateway, In-basket, etc…). Perform timely triage, providing symptom management to patients in collaboration with the care team utilizing an established plan of care. Communicate changes in patient status to the care team (providers and treatment R.N.) in a timely manner (other team members as needed) In collaboration with the scheduling team, assists with treatment scheduling. Collaborate with Charge/Resource Nurse for urgent/same-day infusion appointment(s) when needed. In collaboration with New Patient Coordinator, reviews new patient referrals for timely coordination of care and/or scheduling. Assure patient treatment consent is signed. Consistently follow policies and procedure for telephone/verbal orders. Encourage mutual respect, trust among co-workers, fostering open communication Make appropriate referrals to LICSW, Nutritional Services, Hospice, home care, and/or patient support groups as needed. Act as a liaison between the Cancer Center and other patient care sites and/or pharmacies. Complete prior authorizations, co-pay assistance applications, and specialized testing requisitions as needed. Support provider, nursing and department goals in providing a positive, creative, supportive environment. Participate in and supports strategic planning for the Cancer Center. Current Registered Nurse licensure in the State of Massachusetts Baccalaureate degree from approved Nursing School Program preferred Oncology Certification (OCN) preferred 2 or more years of ambulatory nursing experience required Effective experience with multidisciplinary teams required 1-2 years of Oncology Nursing experience preferred Must be energetic, organized, and process focused to coordinate the complex care and symptom management of oncology patients.
