Position Objective:

Responsible for carrying out Clinical Pharmacy services with primary practice in the area of critical care. Promotes evidence-based pharmacotherapy and sound pharmaceutical care through the development of medication management protocols, and drug therapy policies. Serves as a Critical Care Clinical Pharmacy resource for the health system.

Essential Job Duties:

  1. Conducts departmental activities to meet the goals, objectives and policies of the pharmacy department.
  2. Participates in improvements in interdepartmental policies and procedures, which support the Pharmacy Department and Hospital's strategic goals. Maintains accurate records as required by departmental policies and procedures and/or federal and state board regulations.
  3. Completes all core competencies and documents clinical activities accurately and according to policies and procedures. Supports the Investigational Drug Service in the daily operations of the department.
  4. Serves as the medication expert on intensive care and intermediate care units and is available to provide time-sensitive, accurate information for other healthcare providers in acute scenarios.
  5. Provides guidance for critical care drug selection and pharmacotherapy regimens in accordance with evidence-based guidelines. Plays a vital role in resuscitation efforts during medical emergencies.
  6. Completes pharmacotherapy drug class reviews, formulary addition/deletion, assimilation of evidence-based protocol/order-sets, medication safety related activities and/or targeted medication expense reductions.
  7. Assists in appropriate transition of care services, including medication reconciliation and hand-off of high acuity ICU patients (or those with pertinent medication-related problems).
  8. Serves as the pharmacy representative on multidisciplinary committees involving critical care and develops critical care-specific educational programs, protocols, standard operating procedures, and order sets.
  9. Educates other healthcare providers, via formal and informal in-services, on approved treatment guidelines and criteria to increase appropriate and cost-effective use of medications in the ICU/IMU.
  10. Serves as a preceptor for Pharmacy School Students and AAMC Pharmacy Residents.

Working Conditions, Equipment, Physical Demands:

  • Light work. Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly to move objects. If the use of arm and/or leg controls requires exertion of forces greater than that for sedentary work and the worker sits most of the time, the job is rated for light work.
