Job Summary

Prescribes, delegates, and coordinates pharmaceutical care based upon the patient's diagnosis, age, dietary, and psychological needs. These activities require collaboration with other members of the health care team to provide optimal care.

Minimum Qualifications

Minimum Education: B.S. or Pharm D in Pharmacy from an accredited college of pharmacy
Minimum Years of Experience: 2 Years of oncology pharmacy experience
Substitutable Education & Experience: None.
Required Certifications/Licensure: Licensed as a pharmacist for the State of South Carolina
Required Training: Proficient with computers;

Hospital yearly in-services.

Essential Functions
  • Establishment of guidelines for admixture of chemotherapy agents. 
  • Admixture or supervision of admixture of all chemotherapy agents.
  • Reviewing and reporting costs of chemotherapy agents by patient utilizing EPIC.
  • Quality Assurance/Improvement of the pharmacy oncology service.
  • Maintaining an accurate and in-date chemotherapy inventory.
  • Assuring proper storage and handling of chemotherapy agents.
  • Assuring OSHA standards are maintained.
  • Investigation drug protocols.     
    • Review of protocols with recommendations to the IRB/Ethics Committee of LMC. 
    • Working with the clinical trials nurse in implementation of protocols.   
    • Creating and/or reviewing protocols in EPIC.    
    • Maintaining accurate records of patients receiving such agents utilizing clinical trials functionality of EPIC.
    • Other requirements on an as needed basis.
  • Follow-up on all adverse drug reactions.
  • Medication Error reporting program with recommendations for improvement.
  • Education of nursing staff on pharmacology issues unique to oncology and specific drugs administered in the practice.
  • Review and approve all new drugs to be administered.
  • Development of pharmacoeconomic models for demonstration of cost savings in clinical activities.
  • Maintenance of Pharmacy reference materials, including recommendations for software.
  • Participation in weekly cancer conferences including Tumor and Breast.
