• Shift: Rotating, Saturday 600-1800 and Sunday 600-2000 (26 Hours)-- Open to 3 x 12s, please call this out when subbing to the position if your traveler wants 3 x 12s.

• Unit Information: We are an outpatient facility the services an ED. We do not have inpatients or surgery

Additional Details
• Valid Driver's License or Passport accepted. Must be a clear copy.
• Online Verification of License to include expiration date and any disciplinary actions (within 30 days before start date). Must be through primary source. IN.gov.
• LICENSING: Task the Program Manager for approvals on license issues.
• Traveler must follow link and complete COVID Survey prior to start. Once Complete, submit a screenshot (MyAya).
• CCS: Please collect if the candidate is unable to use MyAya. Must have it on file for clearance. Do not clear without a screenshot on file.
