The Holyoke VNA is looking for an#Outcomes Coordinator to join the team. This#position requires the ability to critically think and independently coordinate the OASIS assessment mentorship program as well as critically evaluate OASIS outcomes and provide insight into improvement opportunities. The work is varied and requires the individual to coordinate between agency outcome evaluation, clinician OASIS training and evaluation, scheduled and creative mentorship methods, and direct patient care. The individual will be supported by the Director of Quality with the goal of increased independence in these areas.# Responsibilities Develop and maintain OASIS expertise through continuing education as well as discussion with the Director of Quality.# Coordinate OASIS Mentorship program. Perform preliminary OASIS in-house training for, subsequent in-the-field OASIS assessment with, and post-assessment evaluation (including inter-rater reliability) and education of all current and new clinicians. Perform quarterly evaluations of clinicians# OASIS outcome competency with development of individual clinicians# training plans. Work with Clinical Operations as needed to schedule out OASIS training and SOC assessments with designated clinicians.# Maintain organized spreadsheets of each clinician with Outcome Areas to Improve, Outcome Goals, and Individualized Training Plans. Meet weekly with the Director of Quality to evaluate the training program. Evaluate agency OASIS outcomes and high functional impairment rate monthly. Perform monthly quality chart audits as needed, to further evaluate quality outcomes. Meet monthly with the Director of Quality to present monthly OASIS outcomes and evaluate agency outcomes improvement plans. Perform OASIS and routine visits as needed, independent of OASIS Mentorship program. Maintain a limited caseload of clinical patients. Perform OASIS#in-the-field assessments as part of the Mentorship program on weekends as needed, to adequately train weekend clinicians. Minimum Requirements Licensed in the state of MA Bachelors degree preferred #Three (3) to Five (5) years of nursing or therapy experience with at least one (1) year experience in Home Care #CPR Certified Required #
