POSITION SUMMARY

To be fully engaged in providing Engagement, Quality/No Harm, Customer Experience and Stewardship by: Ensure the appropriateness of services, orientation of new associates, and education of staff. Conduct field supervision of associates, and supervision of direct reports, regardless of level – RN, LPN, C.N.A., HHA, or Companion to ensure customer delight, quality outcomes and industry compliance. Assist in business development of self–pay market opportunities as well as the medical staffing business line. Represent Health First Private Duty Nursing in all aspects of role and out in the community, and especially when attending recruiting functions as needed.

PRIMARY ACCOUNTABILITIES

  1. Assist with competency evaluations of clinical staff and provide input for 90 day and annual evaluations.
  2. Review appropriateness of services with every referral and assist in all the Admissions/Recertification Plan of Care process.
  3. Perform in home supervisory visits for all clinical associates annually and as needed throughout the year and submit report to Manager.
  4. Coordinate patient care by facilitating communication between physician, insurance companies, pharmacies, staff and other ancillary services Comply with State and Federal Regulations in reference to delivery of Private Duty Home Care Services.
  5. Ensure competencies for all Clinical Staff are administered and evaluated annually. Assist with all clinical associate education, including but not limited to, on–boarding, orientation, clinical assessments, implementation and patient management. Ensure compliance with on–going education required by Health First and/or other guidelines and assist with non–clinical education if needed.
  6. Perform concurrent and retrospective clinical record review to ensure all services and outcomes are documented and that documentation follows the Plan of Care.
  7. Perform rotated clinical on–call after hours and weekends with Clinical Manager, and as needed.
  8. Assist with patient concerns with other members of the health care team; investigate and help resolve patient complaint.
  9. Coordinate scheduling coverage with Business Office Coordinators as needed and/or support during peak census times and vacations Ensure proper documentation guidelines are followed for appropriate reimbursement and regulatory compliance.
  10. Coordinate case management with insurance company representatives to ensure eligibility for services, pre–authorization of care, and ongoing authorization of care as required by insurance company to facilitate payment of claims.
Qualifications

MINIMUM QUALIFICATIONS

  • Education: Graduate of an accredited school of Professional Nursing.
  • Work Experience: One (1) year Home Health nursing or previous clinical supervisory experience considered.
  • Licensure: Current licensure in the State of Florida as a Registered Nurse.
  • Certification: Must hold current American Heart Association (AHA) BLS Healthcare Provider completion card prior to start date and maintain certification; eligible Level II background screening.
  • Knowledge/Skills/Abilities:
    • Knowledge of AHCA regulations for homecare necessary.
    • Communication skills ? auditory, written and verbal.
    • Flexible; adaptable to work in varied environments, such as patients? homes, or living facilities.
    • MS Office Suite and other related software or systems related to role and/or industry.

PREFERRED QUALIFICATIONS

  • Education: Bachelor?s of Nursing
  • Work Experience: One (1) year private duty supervisor role; or Two (2) years acute care RN experience with current skill and competency.
  • Knowledge/Skills/Abilities: IV skills

PHYSICAL REQUIREMENTS (ADA, Travel, Environment, Noise)

  • Visual hand–eye coordination.
  • Lift, pull, or reach for heavy objects up to 15 pounds.
  • Lift, or maneuver patients with or without a supportive device.
  • Frequently stand or sit in front of a computer for a minimum of eight hours a day.
  • Perform frequent travel in all types of weather conditions, throughout county to enter patient homes, facilities, and hospital settings.
  • Must be able to climb stairs if elevator not available.
