Supervises the coordination of a wide variety of administrative, operational and technical activities to ensure safe and efficient Hackensack Meridian Health Transfer Center operations. Facilitates communication between sending and receiving facilities, including the use of telephone, computer software, and other communication and documentation tools. Documents a record of the encounter. Utilizes clinical expertise, critical thinking, and demonstrated knowledge of unit-specific admission and discharge criteria.

  1. Assumes 24-hour responsibility for all matters as they relate to departmental business and operations.
  2. 2. Fill in for Manager in times of absence. Make administrative decisions, hold meetings, ensure timekeeping and payroll procedures and provide accountability.
  3. Assist in mandatory reporting to local administrative leadership as well as network personnel.
  4. 4. Responsible for quality assurance and team member feedback.
  5. Assist with the coordination of team meetings.
  6. Participate in recruitment, onboarding and separation processes.
  7. Represents HMH Transfer Center on appropriate committees, task forces, professional organizations in a manner which reflects positive, progressive and collaborative growth.
  8. Communicates regularly with peers, superiors and subordinates on issues regarding Transfer Center workflow and protocols, network updates, and new procedures.
  9. Help create and foster a respectful and inclusive team environment.
  10. 10.Self-motivated, independent, but seeks guidance from senior leadership when needed.
  11. 11. Answers telephones in accordance with established policies and procedures.
  12. 12. Utilizes professional tone and demeanor in all communication.
  1. Performs all tasks in accordance with Clinical Coordinator RN and Transfer Center Agent job descriptions.
  2. Patient age specification: Neonate, infant, toddler, pre-school, school age, adolescent, adult, geriatrics.
  3. Must be able to sit and work at a computer with multiple monitors while speaking on a telephone for extended periods of time.
  4. Other duties and/or projects as assigned.
  • Current and valid New Jersey Nursing license is required
  • BLS/CPR certification by American Heart Association is required within 60 days of hire
