The Out-Patient Rehab Care Coordinator is a member of the healthcare team and is responsible for coordinating, communicating and facilitating referrals and providing resources in the areas of transportation, outpatient therapies, home therapies, home health, support groups and other community resources The out-patient Rehab care Coordinator will work with the out-patient departments including the medical office and out-patient therapy departments to identify barriers to current care and treatments plans and provide assistance and supportive services to help mitigate those barriers. This position will report to the Inpatient rehabilitation rehab care coordinator supervisor.
Education, Knowledge, Skills and Abilities Required:
1. MSW from an accredited school of social work.
2. Excellent written and verbal communication skills.
3. Proficient computer skills that may include but are not limited to Microsoft Office and/or Google Suite platforms.
Education, Knowledge, Skills and Abilities Preferred:
1. Prior medical social work experience.
Licenses and Certifications Required:
1. NJ Licensed Social Worker or NJ Licensed Clinical Social Worker.
