- Active, unrestricted, Arizona RN License or valid, participating compact license- Required

- DL or State Photo ID always required. Passport not accepted alone

- If using US passport card on the I-9, a copy of the front AND back of the card is required
- Traveler will float to any of the following MS TELE units: 2 East- Cardiac/Observation Unit, 3 West - Stroke Unit, Kendrick- Ortho/Trauma and Total Joint Unit, 3 North-Orthopedic Spine Unit, Humphrey-Palliative/Peritoneal Dialysis/Thyroid Ablation Unit, Cardiac Monitoring Room
