Description
Fairview Health Services is seeking a Family Medicine Physician to support Same Day services at our primary care practices at M Health Fairview Clinic – Rogers and service our M Health Fairview Acute Diagnostic Center - Maple Grove. This position will split its FTE between the two locations, with 0.8 FTE at M Health Fairview Clinic - Rogers and 0.2 FTE at M Health Fairview Acute Diagnostic Center - Maple Grove.
The Acute and Diagnostic Services team is a member of the primary care health care team who works in collaboration with providers and other medical staff to provide acute and diagnostic services to stable patients that are not available or feasible in a primary care clinic. The role of an ADS Physician is to provide assessment and treatment for stable patients who require services that are more complex, require a longer intervention, or are services that are not provided in a typical primary care or urgent care clinic - but do not require emergency department stabilization services. Patients are referred internally to the ADS from Fairview Urgent Care or Primary Care Clinic locations.

PRACTICE DETAILS

- 8 provider primary care
- Patient encounters average 20-22 patients per day.
- Openings: up to a .8 FTE - open to a lesser FTE
- approximately 27 patient contact hours per week
- As a Pioneer Accountable Care Organization (ACO), our physicians use a multidisciplinary team approach in managing clinical care.
- Fully integrated Electronic Medical Record (EPIC).
M Health Fairview Acute Diagnostic Center - Maple Grove
- Openings: 0.2 FTE
- ADS Hours: Monday - Thursday 9:00am - 6:00pm; Friday 9:00am - 5:00pm
- Care for patients ages 16 and up
- No Call required.
- Fully integrated Electronic Medical Record (EPIC).

Qualifications:- Graduate of an accredited ACGME/AOA Family Medicine residency training program.
- BC/BE with the ABMS or AOA.
- Licensed or ability to obtain licensure through the Minnesota Board of Medical Practice.
- Ability to obtain and /or maintain DEA certification for the State of Minnesota.
- Meet threshold criteria/qualifications for Credentialing and Privileges (if required).
