LTC RN Case Manager - Seattle, Washington
- Start Date: ASAP
- Shift: 5x8hr Days (Mon-Fri; Weekends not required)
- Years of Experience REQ: 1 year of prior experience in LTC or geriatric nursing REQUIRED (med surg or rehab medicine experience preferred)

SPECIAL REQUIREMENTS
- Candidate is expected to have reliable transportation and no additional scheduling restrictions. All travel pairs will be required to have their own transportation.
- EPIC experience is REQUIRED.
- The Registered Nurse Case Manager establishes a well-rounded care practice that includes: a patient assessment, care planning, implementation, coordination, and monitoring and ongoing routine evaluation that ensures medically and financially appropriate care. Responsibilities include: facilitating interdisciplinary team care coordination, intensive medical management of participants recently discharged from hospital and skilled nursing facilities. Additionally, the RN CM will work closely with the IDT Social Worker and Physician, to collect the necessary medical and social information to ensure comprehensive decisions are made which ensure the safety and wellbeing of participants.
