Job Summary:
The Population Health Program Specialist is responsible for coordinating system-wide community health initiatives within the Englewood Health broader community. They are an integral member of the Population Health team and provide front-line leadership and support for outreach programs. The Specialist will be responsible for developing new community based relationships and coordinating, implementing, managing, and evaluating community wellness programs. They will be accountable for helping to spearhead new programming, day to day operations of these programs and must possess the ability to manage relationships, multi-task, problem solve, and pay attention to detail. Multi-organizational networking and collaborative partnership building are key features of this role. Other duties may include identifying and implementing opportunitites to engage community members in educational/preventive care programs. The employee must have strong analytic and communication skills and the ability to converse comfortably with community members, caregivers, practice and program staff, administration, and community leaders.

Education Requirements:
- Bachelor's Degree required
- Masters of Social Work, Community Health, Public Health, or health related field preferred
License/Certification/Registry:
- Current driver's license required and access to a vehicle
