Position Summary

  1. Community Liaison Entry Level
  2. Maintains an in-depth knowledge of the community, health care facilities, and referral sources appropriate for agency services.
  3. Assists the Marketing and Business Development Leadership team in establishing targets and goals for the development of ALL service lines in the agency.
  4. Responsible for establishing, growing, and maintaining relationships with facility-based referral sources, in accordance with Company policies and procedures, by both communicating with existing referral sources and identifying new opportunities.
  5. Works with new clients and families on current and future needs for care and obtains consents and contracts as requested.
  6. Remain current on HHA education and competencies to assist with patient care as needed.
  7. Provides in-services and professional presentations for referral sources, community groups and other organizations, regarding DAH's services, programs, and outcomes.
  8. Maintains regular contact with building partners, including but not limited to DHW, ED's, Activity Directors, Resident Service Directors, and care staff.
  9. Assesses the patient's clinical needs and gathers clinical information from the hospital, ALF, family, and the patient and provides to intake department.
  10. Communicates accurately and completely to physicians, staff members, patients, and families, utilizing positive interpersonal skills.
  11. Completes all required documentation on correct forms with accuracy, timeliness, legibility, and professional terminology.
  12. Champions the mission and vision of the organization and projects a positive and professional image for the agency in the community.
  13. Assist with tasks and activities as assigned by Directors or Administration.
  14. Attends all patient care conferences as scheduled.
  15. Develops and maintains a retention/wellness program for team members within the communities served.  

Qualifications

  • Must understand healthcare operations, legal guidelines, competitive analysis, and marketplace trends.
  • Must have reliable transportation, valid Florida driver's license and be able to provide proof of automobile insurance. 
  • Experience in a home care and/or health-related field preferred.
  • Ability to set priorities; competent organizational skills.
  • Ability to handle stressful situations/deadlines.
  • Excellent oral/written communication, presentation, and interpersonal skills.
  • Must demonstrate the ability to communicate effectively with all members of the health care team.
  • Demonstrates desire to work in a business development role.
