Description

Works from a multi-line centralized communication center, offering triage and health information to patients by prioritizing incoming calls based on clinical acuity. Responsible for assessing the needs of the patient, developing a care plan based upon phone triage assessment, and directing patients to the appropriate healthcare provider/facility if required.

Responsibilities:

Provides patient education regarding disease management and medications, and anticipatory guidance and counseling.

Provides healthcare recommendations based on assessment of patient needs using approved protocols.

Processes prescription refills

Assesses patients by eliciting a focused health history.

Documents accurately all call inquiries, communicates complex triage calls with Director and/or physician, and takes verbal orders.

Assists with statistical data collection

Provides healthcare information to patients through telecommunications media using the nursing process.

Communicate with other departments and pharmacies regarding patient needs, as necessary.

Orders labs/testing and schedules appointments with the office as neededKnow and follow all organizational/departmental policies/procedures

Certificate in Nursing required.

3 years of experience as a Staff RN required.

Licensure, Certifications, and Clearances:

Cardiopulmonary Resuscitation (CPR)

Act 34

Registered Nurse (RN) - Registered Nurse licensure in the Commonwealth of Pennsylvania required

Current licensure either in the state where the facility is located or, if the facility is in a state covered by the multistate Nursing Licensure Compact (NLC) agreement, a multistate license issued by a participating NLC state. Hires and current employees working on an out-of-state NLC license who later change their residency to the state where the facility is also located will have 60 days upon changing their residency to apply for licensure within that state.
