The primary role of the Nurse Supervisor is to provide assistance to the Director of Nursing with oversight and management of employees whom provide coordination and infusion nursing services to patients serviced by Optum Infusion Services pharmacy. The Nurse Supervisor will oversee coordination and manage the nursing activities of other nurses in an assigned area with the Director of Nursing. These nurses may be a combination of contract HHA nurses and / or direct employees of Optum. Service delivery will occur in patient's home or alternate site per patient's request. All infusion services and medication delivery will be provided utilizing accepted infusion therapy clinical technique and Optum's established policy and procedures ordered by a licensed physician or licensed practitioner. All clinical operations will be consistent with established goals, objectives, and policies, and specifically focused on delivery of infusion therapy nursing and medication delivery.
**Primary Responsibilities:**
Ability to establish and maintain positive clinical rapport with all patients and their families, representing OptumRx including working directly with patients and their families, maintain contact and clinically monitor all patients providing direct infusion therapy services
Oversight and supervision of nursing intake referral process and team, address issues that arise and manage and/or escalate as appropriate
Infuse medications as needed in patient's home setting including performing procedures such as IV placement, phlebotomy, and medication administration via peripheral infusion device; or PICC line; Implantable port; and / or Central Venous Catheters and as ordered by a licensed physician or licensed practitioner
Assess patient in home environments, including clinical assessment, monitoring and tracking vital signs, safety of environment, pain assessment, monitor effectiveness of medication treatment, develop plan of care, education for patients and patient's family members, including providing care
Perform supervisory visits for assigned nursing personnel
Establish and maintain all clinical documentation including, but not limited to, Plans of Care/Treatment (POC/POT), Clinical Summaries, medication changes and any updates for OptumRx pharmacy
Communicate effectively to internal OptumRx personnel, any patient status changes or information relevant to promote the coordination of care and safety of a patient
Willingness to work with most ages and diagnosis of patients receiving infusion therapy medication
May work with numerous infused or Injectable medications including orals per OptumRx protocols
Clinical oversight of patient functions provided by OptumRx Infusion Services, within a limited geographical area
Collaborative relationships involving agencies delivering services on behalf of / or in conjunction with OptumRx, including nursing agencies in a contracted relationship, patient, families, significant caregivers, and other healthcare team members, in a limited geographical area, if needed
Maintaining current State Nursing licensure, as required by the State Department of Health, is mandatory
Assist in securing and maintaining all licensure and accreditations warranted by OptumRx, including required Home Health Licensure, ACHC and any other licensure or credentialing as necessary
Compliance and adherence to all State and Federal regulations and laws, ACHC, OSHA, HIPAA, CPR, all infusion pharmacy and nursing licensure accepted standards of practice
Maintain all mandatory education required upon hire and annually per policy
May be required to represent the company in clinical interactions as requested
May provide education regarding OptumRx Infusion Services, for both internally and external customers
Function as clinical educator and resource
May be required to work any day or shift as required to accommodate patient schedules in administering medications, on occasion
May be required to lift or carry equipment and supplies to patients home
May be required to participate in after hours on call responsibilities, on occasion

**Required Qualifications:**
Active and unrestricted licensure as a Registered Nurse (RN) in the state of California
Current Basic Life Support (BLS) and/or CPR certification, or willing to obtain prior to starting employment
3+ years of experience working as an RN in a clinical care setting that contributes to proficiency in infusion therapy (such as medical surgical unit, emergency department, intensive care unit, post-anesthesia care unit, or similar setting)
1+ years of management experience; team lead or preceptor experience will also be considered
Knowledgeable and/or experienced with current state licensure requirements
Knowledge of the Infusion Nursing Standards (INS) and nursing process including assessment, problem identification or nursing diagnosis, outcome identification, planning and communicating, implementation, and evaluation
Current knowledge of Vascular Access Devices (VAD's)
Excellent IV skills
High level of proficiency with the use of technology, through exposure to EMR, use of data phones or tablets with WiFi access, and proficiency in Microsoft Office Suite (Excel and Word)
Proven excellent interpersonal, listening, organizational, and critical thinking skills
Proven ability to work with an interdisciplinary team of professionals including clinical, pharmacy, physicians, outside infusion nursing agencies, patients, families, and significant caregivers
Proven professional appearance and teaching skills
Proven ability to work independently in home or alternate site settings
Proven ability to work under pressure with clinical emergencies if required
Ability to meet attendance, overtime, on-call, and other reliability requirements of the job
