Summary:
Come join a growing PACE team and make a difference in the community! PACE (Program of all Inclusive Care for the Elderly) is a unique model that provides medical care and social support to patients 55+ with complex medical/social needs. The goal of the PACE program is to assist patients with remaining in their homes as safely and as long as possible. This program only serves individuals in Allen County. The PACE center is located at 2927 Lake Ave and houses a day center and therapy gym in addition to a medical clinic. The PACE Homecare RN would oversee the clinical care in patient homes which is a key component of assisting these patients to remain in their homes. The PACE Homecare RN will work directly with the PACE providers (who are located at the PACE clinic daily and also provide in-home support) and a comprehensive interdisciplinary team. A typical day includes in home patient assessments (both routine and as needed), medication reconciliation and education, would care, diabetic education and oversight, and general coordination/education with the patient and their caregivers.
Other clinical skills include IV antibiotics, catheter management, and laboratory specimen collection. The PACE model encourages and promotes teamwork amongst many disciplines with an emphasis on creative patient solutions to improve overall quality of life. Schedule: The PACE program is open Monday-Friday 8:00-4:30PM with flexible on call schedule that is primarily telephonic. On Call will decrease as the program grows.

Qualifications:
Requires a Diploma in nursing or an ASN/BSN
Valid Indiana Registered Nurse license required. CPR certification.
Additional certifications may be required for specified areas.
Must be able to lift 50 pounds, assist with 100 pounds. (Lifting requirements may be heavier for specified departments.) Must have good verbal and written communication skills.
