Description

Job Description:- Provide strong leadership to the nursing team, fostering a positive and collaborative work environment. - Supervise and evaluate the performance of 2 RNs and 4 CNAs, ensuring adherence to facility policies, procedures, and best practices. - Develop and maintain staffing schedules to ensure adequate coverage and efficient patient care delivery.- Collaborate with the interdisciplinary team to develop and implement individualized care plans for residents. - Monitor the overall health and wellbeing of residents, ensuring that appropriate medical interventions and treatments are provided. - Conduct regular rounds to assess patient conditions, address concerns, and identify opportunities for improvement.- Implement and monitor quality assurance programs to ensure compliance with state and federal regulations, as well as facility standards. - Participate in regular audits, reviews, and surveys to maintain a high standard of care and service delivery. - Analyze data and performance metrics to identify trends and implement strategies for continuous improvement.- Maintain open lines of communication with residents, their families, and the interdisciplinary team to ensure that care plans are well-coordinated. - Collaborate with physicians, therapists, and other healthcare professionals to optimize resident outcomes. - Act as a liaison between the nursing team and facility leadership, conveying information, updates, and concerns as needed. Qualifications:- Current and valid Registered Nurse (RN) license in the state.- Bachelor's degree in nursing (BSN) preferred. - Proven experience in nursing leadership or management roles within a SNF or similar healthcare setting.- Strong understanding of long-term care regulations, standards, and best practices.- Excellent communication, interpersonal, and problem-solving skills.- Ability to work collaboratively within an interdisciplinary team. - Proficiency in using electronic health record (EHR) systems and other relevant healthcare software. - Demonstrated commitment to providing compassionate and patient-centered care.- Effective organizational skills and attention to welcome applications from qualified candidates who have passed the NCLEX licensed in NY.
