**Summary:**

Kidney Center - must either currently possess CPN or obtain within 6 months of eligibility to sit for exam - Mon-Fri 8am-4:30pm

Provides safe, effective, and holistic patient and family centered care for the patients and families in Primary Care Services. Leads clinic efforts in patient-centered medical home related population management, data collection and quality improvement activities. Addresses the patient's preventive care and care coordination needs within a specialized area of clinical practice. Acts as a clinical resource for patients, families, staff, and community providers.

Responsibilities of the **Care Coordinator** for the Center for Kidney and Urinary Tract Disorders (aka Kidney Center) include the following:

**Care Coordinator for Multidisciplinary Clinics within the Kidney Center**

Providing direct patient care within multidisciplinary clinics (e. g., Kidney Stone, UroNeph).

Conducting assessments to ensure patient comfort and safety.

Collaborating closely with physicians and healthcare professionals for comprehensive patient care.

**Care** **Coordinator for** **fetuses with** **Congenital Anomalies of** **the** **Kidneys and Urinary Tract (CAKUT)**

Managing prenatal care coordination for expectant mothers.

Scheduling appointments and providing patient education.

Collaborating with obstetricians and other healthcare providers to optimize maternal and fetal health outcomes.

**Care Coordinator for** **Kidney Transplant Candidates and Recipients** **with urologic disorders**

Coordinating urological care throughout the transplant process (evaluation, listing, post-transplant)

Managing evaluations, tests, procedures, and post-transplant follow-up care.

Ensuring continuity of care to support patient recovery and well-being.

**Coordinator for** **Radiology/Urology/Nephrology (RUN)** **Case Conference**

Organizing meeting agendas and assembling monthly patient lists.

Keeping detailed meeting minutes.

Overseeing conference operations to ensure smooth execution.

Facilitating networking and collaboration among healthcare professionals.

**Clinical Coordinator for** **a** **select** **cohort of** **Spina Bifida** **patients**

Coordinating care and support services for spina bifida patients requiring nephrology and urology care.

Facilitating communication between patients, families, and healthcare providers.

Developing comprehensive care plans tailored to spina bifida patients' unique needs.

**Care Coordinator for** **UDS (Urodynamic Studies)**

Facilitating urodynamic studies by coordinating appointments and preparing patients.

Managing test results and communicating findings to patients and healthcare providers.

Coordinating necessary follow-up care based on test outcomes.

**Community Outreach** **Coordinator**

Collaborating with the multidisciplinary team and community providers to increase patient referrals.

Developing and implementing outreach strategies to raise awareness of clinical services.

Working in tandem with AC tocreate marketing materials, manage social media platforms, and participating in community events to engage potential patients and referring providers.

**Required Education:**

Associate's degree or equivalent from two-year college program or technical school or nursing diploma, Associate Degree Nursing

**Description**

1. Coordinates continuity of patient care with patients and families following hospital admission and discharge, and for clinics ER visits, and PCP visits. Facilitates hand-off throughout the continuum of care.

2. Manages high-risk patient care, including management of patients with multiple comorbidities or high risk for readmission to a hospital setting.

3. Provides and facilitates education of parents/families of patients following admission and coordinates discharge planning.

4. Serves as resource for team promoting advocacy for patients, families, and Arkansas Children's Team.

5. Conducts comprehensive, preventative screenings for patients and/or assists all support staff in daily patient interactions as needed.

6. Consistently provides safe and effective nursing care at a proficient level.

7. Proficiently assesses increasingly complex patients and serves as a resource to others to ensure developmentally appropriate and culturally sensitive care.

8. Provides individualized patient and family education at a proficient level in increasingly complex care situations, and serves as a resource in guiding others.

9. Proficiently mentors, coaches, precepts, and teaches healthcare professionals and students in the care of increasingly complex patients.

10. Performs other duties as assigned.
