Job Title: Case Manager (RN)Job Description
Develops, assesses, and facilitates complex care management activities for members with primarily physical needs to provide high-quality, cost-effective healthcare outcomes. This includes creating personalized care plans and educating members and their families. Evaluates member needs, identifies barriers to accessing care, and recommends plans for optimal outcomes. Develops ongoing care and service plans, collaborates with providers, and coordinates care to ensure members receive person-centered services. Monitors member status and revises care plans as necessary. Provides resource support and facilitates education on disease processes, care options, and healthcare benefits. Collects and maintains member information to ensure compliance with regulations. Provides feedback to improve care quality and performs other duties as assigned.
Hard Skills
Degree from an Accredited School of Nursing or Bachelor's degree in Nursing
RN - Registered Nurse - State Licensure and/or Compact State Licensure
2-4 years of related experience
2 years' acute RN experience
NY RN licensure
