Registered Nurse - WellSpan at Home - Day

Location: WellSpan Medical Group, York, PA

Schedule: Full Time

Schedule and Location

Full Time: 40 Hours/Week

Hours: Rotating Shifts - 8am - 6pm / 9am - 7pm / 10am - 8pm

Every 3rd Weekend 8am - 6pm

Every Other Holiday Required

Shift and Weekend Differential Pay Eligible

Hybrid role that will be able to Work from Home when not working in Patient Homes!

General Summary

Provides professional nursing care at the intermediate or expert level in accordance with the principles of patient and family-centered care based on the nursing process and according to the clinical standards of the hospital. Coordinates care planning with underdisciplined. May requires specialized skills depending on assignment. This position will support the WellSpan at Home program with the hours divided equally among the Remote Patient Monitoring and Hospital at Home program groups. As needed, will provide support to Advanced Illness Management and Primary Care at Home.

Duties and Responsibilities

Essential Functions:

Hospital at Home Duties and Responsibilities (Occurs in the patients home):

Independently completes assessments and collects data on assigned patients according to policy; assesses situations as complexity arises. Develops innovative approaches to solve complex patient care concerns. Focuses on important parameters and emerging problems to predict and manage an evolving clinical picture. Establishes contingency plans in anticipation of unexpected events and coordinates emergency action in response to acute risk situations. Demonstrates leadership at the point of care by actively preforming caring behaviors with every patient interaction.

Demonstrates advanced knowledge and skills to administer prescribed treatments, medications, and nursing interventions safely and independently for more complex groups of patients based on assessment and plan of care. Provides supervision and acts as a resource person for all expected unit skills and equipment. Independently organizes and prioritizes time based on higher acuity patient assignments and increased workload.

Independently evaluates and monitors the effectiveness of interventions and progress toward outcomes for more complex groups of patients; analyzes data to make recommendations for changes in the plan of care. Initiates and coordinates interdisciplinary collaboration in evaluation of patient progress. Anticipates and coordinates individualized patient needs for transfer/ discharge and includes patient/family in decisions.

Independently reads current literature. Demonstrates inquiry to improve patient outcomes. Participates with a team to evaluate evidence and determine clinical relevance. Coaching and mentoring the use of EBP and Peer Nurse Review at the unit level using current professional literature and research to improve care at the bedside.

Seeks learning opportunities and resources beyond own work environment and incorporates into practice the new knowledge and skills acquired. Completes required education and competencies. Shares pertinent information and job knowledge, related to readings, educational programs, and participation in professional organization activities.

Collaboratively works with management to monitor the level of professional practice on the unit; assists with the initiation of actions or programs to ensure teamwork and professional practice. Serves as a problem solver to improve unit and hospital outcomes.

Mentors/coaches others to improve decision-making and accountability. Serves as a resource to nurses developing supervising and delegating skills.

Promotes cooperation and communication among all team members. Anticipates the potential for conflict and works to quickly defuse any situations involving conflict with patient/families and/or co-workers to promote a healthy work environment. Recognizes multiple factors underlying the conflict situation and chooses and initiates actions that result in the most effective long-term solution. In collaboration with management, implements changes to enhance team communication. Coaches team members in developing insights into their own communication patterns.

Remote Patient Monitoring Duties and Responsibilities (Occurs remotely):

Reviews patients for the appropriateness of the Remote Patient Monitoring program.

Reviews biometric data such as vital signs and responses to pathway questions. Determines potential steps including contacting or consulting the patient's provider or specialty provider for further direction to escalate patients care needs.

Obtains physician orders, develops plan of care, and documents findings.

Maintains proficiency in all chronic care conditions including early detection of signs and symptoms of exacerbations, appropriate diagnostic testing, treatments, and escalating care.

Assesses and evaluates immediate and long-term patient needs. Provide education and support per diagnosis including signs and symptoms.

Notifies physician and/or other care teams of changes in patient conditions, sends updates in SBAR format, obtains new or additional orders, and modifies care plan as needed.

Identifies and obtains additional services as needed.

Variation of duties may result based on the assigned customer care unit(s). This may include differences in the specific clinical skills required of the RN. Unit based skill lists outline expectations beyond the house wide RN requirements. SYS-NURS-L Nursing Education Requirements Policy outlines current certification(s) required for specific areas of nursing practice and is considered to be part of the job description.

In addition to requirements specified in the license and certification section below, current BLS certification for all areas of nursing practice is required. Current certification may be required for the following areas of nursing practice:Emergency Care Center - ACLS, PALSIntensive Care Center & MSCC Lead - ACLS, PALSL&D - ACLS, NRP, - Experienced L&D nurses will obtain within 6 months and new L&D nurses will obtain at 1 year of experienceNICU - NRPPEDS - PALSPCC/PACU/ENDO - ACLS, PALSProcedural - ACLS
