**Schedule:** Part Time Weekends

**Shift** : 12 hr shift on Sat/Sun 4 hrs on Monday .....Call Coverage for weekend Friday 5pm-Monday 8am

**Location:** AdventHealth Gordon, 105 Willowbrook Way, Calhoun, GA 30701 servicing Gordon, Whitfield, Catoosa, Murray, Floyd and Bartow.

**The role you'll contribute:**

The agency has a need for weekend coverage that frequently includes a combination of skills to cover essential functions on the weekend that are usually covered by other staff during the week. This may include intake of new referrals; functioning as an "admission" nurse/performance of essential visits for care that must be provided on the weekend, on-call coverage to address unanticipated issues and reduce preventable rehospitalizations, scheduling duties, and serve as an expert resource for other home care staff. Depending on the size of the agency, the weekend nurse may perform some or all of the above functions.

**The value you'll bring to the team:**

- May perform intake function for incoming agency referrals, entering data into the computer, verifying physician orders, assessing payer and patient for appropriateness.

- Functions as an admission nurse, completing admission process for new patients as well as performs home nursing visits as assigned.

- Provides comprehensive assessment, planning, implementation and evaluation of patients assigned.

- Reports changes in patient condition in a timely manner to the physician and/or RN Care Manager or supervisor. Obtains orders for changes in the plan of care to respond to the patient's condition.

- Provides a handoff report to the patient's assigned home care case manager who will be assuming care of the patient.

- Follows orders for treatment and provides services as indicated on the plan of care.

- Utilizes and implements appropriate educational tools and pathways per guidelines and/or policy and procedure

- Takes ownership to optimize agency performance through active involvement in quality improvement activities.

- Utilizes motivational interviewing/health coaching techniques to achieve desired patient outcomes

- Accurately documents and submits timely the assessment, interventions and evaluations pertaining to patient's care management and services in accordance with the agency policy and procedure.

- Implements the plan of care through direct patient care and coordination with other disciplines. Provides care based on physician's orders, in compliance with policies and procedures, standards of care, and regulatory requirements. Provides skilled nursing care, preventative rehabilitative procedures, and prescribed treatments with a variety of patient populations within various potentially complex home situations.

- Demonstrates self-education as evidenced by completion of mandatory annual trainings and continuing educational opportunities. Demonstrates furthering professional growth through sharing knowledge with others.

- Assures that practice reflects current standards and best practice within area of expertise

- Acts independently and responsibly to perform duties on a consistent basis and in a timely manner.

- May be required to attend mandatory agency meetings during the week.

- Participates in Nursing On-Call Rotation as assigned.

Qualifications

**The expertise and experiences you'll need to succeed:**

**EDUCATION AND EXPERIENCE REQUIRED:**

· Minimum of one-year relevant clinical RN experience

**-EDUCATION AND EXPERIENCE PREFERRED:-**

· Recent, relevant experience in a Medicare-certified home health agency as a case-manager

· Bachelors degree in nursing

**LICENSURE, CERTIFICATION OR REGISTRATION REQUIRED:**

· Current Registered Nursing License in State of Practice

· Valid Driver's License and current car insurance

· CPR certified
