**The community you'll be caring for:**

Winter Park Memorial Hospital has continuously served the residents of Winter Park and its surrounding communities for more than 50 years

Chartered in 1951, the hospital has grown from the visionary efforts of a handful of Winter Park residents and community leaders, to a 307-bed acute care facility that is a model of community health and wellness

Over the years the hospital has continually expanded to meet the needs of the community, adding an upscale obstetrics and Level II Neonatal Intensive Care Unit at The Dr. P. Phillips Baby Place, cancer care at the AdventHealth Cancer Institute, Winter Park, and state-of-the-art surgery, recovery and rehabilitation at the AdventHealth Orthopaedic Institute

**The role you'll contribute:**

The Nurse Clinical Expeditor is an effective steward of scarce resources, using clinical expertise, critical thinking skills,

organizational awareness, and leadership to play a pivotal role in enhancing the quality of care, removal of barriers, and

lowering the cost of care. The Expeditor is skilled in coaching, mentoring, and leading a professional and technical team

in the delivery of services to our patient population. The Expeditor partners with physicians and providers, care

management team, nursing teams, and support service teams to facilitate patient throughput and progression of care.

Personal and professional development is an expectation of the role to ensure consistent application of procedures, patient

progression, and optimal communication. Actively participates in outstanding customer service and accepts responsibility

in maintaining relationships that are equally respectful to all.

**PRINCIPAL DUTIES AND JOB RESPONSIBILITIES:**

Consistently demonstrates, through behavior, AdventHealth's core values of Integrity, Compassion, Balance, Excellence,

Stewardship, and Teamwork. Adheres to the ANA Code of Ethics, the ANA Scope and Standard for Nursing Practice, and

the AONE Nurse Executive Competencies.

-Liaises between Care Management, Nursing, and Hospital providers, working collaboratively to solve patient

challenges which includes, but not limited to, financial barriers, religious beliefs, and cognitive issues.

-Incorporates the patient/family care goals and preferences as much as possible into the transition of care planning and

communicates these goals and preferences to the multidisciplinary team

-Follows-up on any outstanding diagnostic test results, pending consults, patient requests or barriers for discharge.

Avoids delay by ensuring test results are available for review and decision making.

-Connects with hospital providers each day to review transition plans on all patients. Identifies patients that have a

discharge delay and identifies barriers to discharge. Works with providers to facilitate safe and appropriate discharges.

Collaboratively manages interdisciplinary patient transition plan. Effectively communicates across continuum of care.

-Proactively identifies patients who no longer meet medical necessity and escalates potential denials, documents

avoidable days, and facilitates progression of care

-Participates in length of stay meetings as appropriate.

-Uses discretion and adherence to policies and procedures when discussing patient related issues that are confidential in

nature. Protects patient privacy with all interactions. Maintains the confidentiality of employees, patients, administrative

staff, functions, and medical staff information with no infractions.

-Actively participates collaboratively with the Campus Leadership including Nursing and Care Management in support

of system goals for capacity management.

-Plays an active role in educating nursing and care management on patient care management from a clinical progression

standpoint.

-Maintains knowledge of federal and regulatory policies in the patient care management.

-Participates in facility and department regulatory and certification preparations.

-Performs other duties as assigned.

KNOWLEDGE AND EXPERIENCE STRONGLY PREFERRED:

-Ability to function in stressful, fast paced environment

-Ability to lead the employees of their unit in the progression of patient care

-Ability to lead performance improvement initiatives on their assigned unit and/or campus

-Ability to assist and mentor other in relation to the technology utilized on the unit

-Excellent clinical practice knowledge and is able to apply this knowledge to the work setting

-Ability to influence relationships and behaviors in relation to meeting the needs of the patient

-Ability to think proactively

Qualifications

**The expertise and experiences you'll need to succeed:**

**EDUCATION AND EXPERIENCE REQUIRED:**

-Bachelors and 2 years experience

-Computer skills that include Microsoft applications and Hospital Electronic Medical Record (EMR)

**PREFERRED:**

-Three to five years of experiences as an RN

-Acute care experience

-Prior leadership experience

-Experience in Case Management, Managed Care, Discharge Planning and Utilization Management

-Case Management training

-Specific knowledge and skills delineated by the unit of work (see competencies)

**LICENSURE, CERTIFICATION OR REGISTRATION REQUIRED:**

- Basic Life Support - CPR Cert

- Registered Nurse
