**Fairview Range is looking for a Registered Nurse to join our Medical Oncology and Infusion Departments at our Mesaba Clinic in Hibbing, MN.**

This position will work as a Registered Nurse Oncology Care Coordinator as well as an Infusion Registered Nurse.

A Registered Nurse Oncology Care Coordinator manifests a high level of expertise to assess, plan and facilitate treatment for actual or potential complex health problems of individuals, families or communities. This individual coordinates and manages the care of patients to meet multiple service needs, ensure continuity of services, decrease fragmentation of care in all settings, enhance the patient's quality of life, and ensure cost containment. The care coordinator assists the patient/family in coping with illness, optimizes the patient's self-care abilities, and supports patient's rights to make choices. In addition to managing patients through an episode of illness, the care coordinator focuses on wellness and disease management. Duties of this position may include the safe administration of chemotherapy agents or other infusions, as well as care for various venous access devices, foleys, and other duties as necessary specific to the patient and treatment plan.

An Infusion Registered Nurse provides direct nursing care to patients as well as indirect care. This may include the safe administration of chemotherapy agents or other infusions, as well as care for various venous access devices, foleys, and other duties as necessary specific to the patient and treatment plan.

**Schedule:** This is a full time position, 72-80 hours per two weeks, 8-hour Day/Evening shifts and weekends as needed.

Specialty departments include surgical services, orthopedics, OB/GYN, ENT, allergy, wound care, podiatry, and more.

Care team includes Physicians, Advanced Practice Providers, RN Care Coordinators, RNs, LPNs, MAs, Patient Access Coordinators

**RN Job Responsibilities**

Timely and accurate assessment of the patient's clinical, social, functional, and continuing of care needs through direct observation, data analysis and interviews in order to evaluate the need for intervention.

Plans for and identifies immediate, short term, and long term care needs for patient/client. Sets appropriate patient/client goals and time frames for achievement in order to achieve desired outcomes.

Evaluates and measures patient/client response to planning, services, and interventions in order to assure quality of case management services and to make improvements where necessary.

Demonstrates ability to communicate appropriate information across service lines to ensure effective and efficient utilization of services.

Performs a total systems assessment according to established policies (including physical, psychosocial and spiritual data).

Demonstrates awareness of age-specific criteria in obtaining assessment data.

Identifies patient needs and/or nursing diagnosis based on analysis of collected data and incorporates into an individualized plan of care.

Analyzes assessment data, identifies problems and develops the plan of care in collaboration with the patient, significant other and resource personnel.

Identifies expected outcomes which are realistic, measurable and attainable.

Provides appropriate nursing care based on assessments and in coordination with medical care, including external referrals, as directed.

Recognized and reports adverse patient condition to the provider in a timely manner.

Safe administration of chemotherapy agents, IV solutions, and medication additives.

**RN Care Coordinator Job Qualifications**

**Required Qualifications**

One year RN experience

Graduate of an accredited school of nursing.

Baccalaureate Degree in Nursing or willingness to obtain within two years of hire.

One year of acute and/or ambulatory direct patient care experience or one year of recent oncology experience.

Current Minnesota RN Licensure.

Current ONS Fundamentals of Chemotherapy certification, or obtains within 6 months of hire

Current ONS/ONCC Chemotherapy/Immunotherapy Certificate, or obtains within 2 years of hire and maintains.

Current BLS certification or attend BLS class within 3 months of hire and maintains certification.

**Preferred Qualifications**

Prior case coordination experience.

One year clinical experience in the Oncology care setting.
