Position: Registered Nurse - Medical Home Port
Location: Jacksonville, FL
Employment Status: Full-time
Compensation: Hourly, paid bi-weekly.
Schedule:
The Primary Care Clinics are open from 6:30 AM to 7:00 PM, Monday through Friday.
Shift lengths may vary (8.5 or 9-hour periods) to include an uncompensated 30-to-60-minute meal break.
No more than 80 hours per two-week period.

Requirements:
Degree/Education: Baccalaureate Degree in Nursing, Graduate Diploma in Nursing, or Associate Degree in Nursing from an accredited agency such as ACEN or CCNE.
Certification: Basic Life Support (BLS), Advanced Cardiac Life Support (ACLS), and Pediatric Advanced Life Support (PALS).
Experience: 12 months of experience within the last 24 months as an RN.
Licensure: Current, unrestricted license to practice as a Registered Nurse in any of the 50 States, the District of Columbia, Puerto Rico, Guam, or the U. S. Virgin Islands.
References: Two letters of recommendation from practicing physicians or peers attesting to clinical skills. Reference letters must include name, title, phone number, date, address, and signature, and must be written within the last two years.
Job Summary:
Coordinate patient care in collaboration with various healthcare professionals.
Facilitate optimal outcomes in clinical care, quality, and cost-effectiveness.
Ensure compliance with standards of care and practice according to MTF policies, procedures, and guidelines.
Perform physical exams and health histories.
Provide health promotion, counseling, and education.
Administer medications, wound care, and personalized interventions.
Supervise care provided by other healthcare professionals.
Assign patient care based on personnel scope of practice and skill level.
Recognize adverse signs and symptoms and react quickly in emergencies.
Communicate and collaborate with a diverse group to benefit patients, families, and the organization.
Arrange specialty care and make referral appointments as needed.
Perform nursing services as outlined in the official Task Order.
Conduct research to support improved practice and patient outcomes.
