**Summary of Primary Function/General Purpose of Position**

The Inpatient Wound Care Registered Nurse (RN) conducts initial and ongoing patient assessment, analyzes assessment data, creates a plan of care, implements treatment, and evaluates treatment effectiveness. They administer medications in a safe manner consistent with the State of Practice and Bon Secours Mercy Health policies and procedures and serve as point of contact for patients' care coordination throughout hospital departments.

**Essential Job Functions**

- Initiates a nursing assessment of patient's condition upon admission or receipt of consultation; identifies the patient's present and potential problems from the assessment

- Recognizes and assesses initial alterations in the patient's body systems and verbalizes this to other members of the health care team

- Assesses, stages, and measures wounds accurately and in accordance with policy.

- Assesses and interprets reports communicating findings with the assigned physician.

- Organizes assessment data for accuracy, completeness and confidentiality; records assessment data in an orderly fashion and communicates, revises, and verbalizes pertinent information to other health care team members; collaborates with physician to develop a plan of care based on assessment data.

- Photographs wounds according to policy and procedure.

- Assesses compliance to the clinical pathway and is instrumental in bringing missing components to the attention of the physician or health care team.

- Identifies patient care situations that require intervention, implements nursing actions, and evaluates responses.

- Identifies patient/family education and discharge needs and implements a teaching plan of care.

- Accurately completes all admission nursing chart forms and documents a plan of care for all assigned patients; determines patients' health status and incorporates it into the plan of care.

- Prioritizes problems according to impact on health status.

- Formulates desired outcomes specific to patient's problems, certain time frame, and protocols; ensures desired outcomes are mutually agreed upon by the patient, family (when appropriate) and the nurse.

- Identifies needs for patient from admission to discharge.

- Coordinates activities with other disciplines to ensure quality of care.

- BSMH nurses are expected to practice and deliver care in accordance with the core concepts and intention of the BSMH Nursing Professional Practice Model.

**Licensing/Certification**

Current RN License in the state in which they are working or covered by compact (required)

BLS Basic Life Support (preferred upon hire, must obtain from approved source prior to direct patient care)

Wound Care Certification (WOCN Wound, Ostomy and Continence Nursing or WCC Wound Care Certification) (required within three years of hire)

**Education**

Bachelors of Science Nursing (preferred)

**Work Experience**

Recent RN experience in acute care and wound care (required)

2 years wound care experience (preferred)

**Training**

EPIC Electronic Health Record (EHR) (preferred)

- Applies cultural diversity and inclusion principles when caring for patients and their support resources.

- Applies knowledge and skills associate with advanced wound care to include, but not limited to, negative pressure therapy, compression therapy, hyperbaric oxygen therapy, the utilization of skin substitutes and total contact casting.
