Schedule: Full Time, Day Shift, 8a-5p with a scheduled 1/2 day off each week. No weekends or Holidays.
Starting Pay: $32.23
Shift Differentials: Available for night, weekend, and additional shifts
Location: SSM Beaver Dam
The selected candidate will be responsible for telephone triage and nursing care assigned in support of medical care as directed by the medical staff. Assists physicians as needed with patients, responds to medical emergencies, triages incoming patient calls, records and communicates information to referring physicians.

**Job Summary:**
Provides direct nursing care in accordance with established policies, procedures and protocols of the healthcare organization.
**Job Responsibilities and Requirements:**
PRIMARY JOB DUTIES
- Triaging both phone calls and patient messages through MyChart, screening and returning patient phone calls. Follow SSM Primary Care Protocols related to: medication refills, referrals, lab orders, lab results, nursing advice and or treatment recommendations. Maintaining accurate EMR documentation.
- Reviewing charts, monitoring patient's progress and lab results as ordered by physicians, recognizing abnormalities and taking appropriate actions as per physician instructions or set guidelines.
- Scheduling tests, procedures as needed, educating and informing the patient or caregiver of the same. Communicating with health care providers regarding orders and well-being of patients.
- Identifying and responding to emergency situations.
- Acting as a role model and liaison, maintaining professionalism in the workplace, collaborating with patients, nurses, staff members, vendors and other health care providers.
- Providing support for coverage in other clinical areas.
- Assisting to orient new staff to the clinic and SSM's policies and procedures
EDUCATION
Graduate of accredited school of nursing or education equivalency for licensing

Constant use of speech to share information through oral communication.

Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs.
Frequent sitting, reaching and keyboard use/data entry.
Frequent use of vision and depth perception for distances near (20 inches or less) and far (20 feet or more) and to identify and distinguish colors.
Frequent use of smell to detect/recognize odors.
Frequent use of hearing to receive oral communication, distinguish body sounds and/or hear alarms, malfunctioning machinery, etc.
Occasional lifting/carrying and pushing/pulling objects weighing 25-50 lbs.
Occasional lifting/moving of patients.
Occasional bending, stooping, kneeling, squatting, twisting, gripping and repetitive foot/leg and hand/arm movements.
Occasional driving.
Rare crawling and running.
