Job Description:

The MDS Coordinator is a Registered Nurse who has the ability to coordinate and organize nursing care related to the resident assessment, as well as conform to CMS guidelines. The MDS Coordinator is responsible for all areas of MDS evaluation and coordination. The MDS Coordinator will ensure that all disciplines included in MDS development and implementation complete the MDS accurately and correctly, following current MDS/RAI guidelines. The MDS RN coordinator utilizes an interpersonal process that uniquely focuses on the total individual and his/her role in their family, culture, and society.

Scope

  • Completes and electronically transmits holistic, age-specific, resident assessment instrument (RAI - consisting of MDS, triggers and CAAs) as dictated by CMS, within the time frames required.
  • Analyzes and incorporates clinical data ( i. e. CAA findings, lab, X-ray, and all diagnostics)
  • Identifies and documents residents at nutritional and functional risk providing oversight of dietary recommendations.
  • Identifies and reports signs/symptoms of neglect/abuse related to child, domestic or elder abuse.
  • Assesses potential admissions for qualifying Medicare skilled criteria.
  • Collaborates with resident/family/significant other and other healthcare disciplines to develop an individualized plan of care based on assessments and MDS triggers within allowed time frame.
  • Coordinates with skilled care providers and Medicare Review Consultants to maximize resident allowable Medicare benefit and facility PPS reimbursement.
  • Communicates with physician to ensure coordination of medical and nursing plan of care.
  • Demonstrates competence in delegating and coordinating with other members of the IDT to ensure accurate and timely accomplishment of the RAI and Care Plan.
  • Schedules and coordinates care plan meetings with residents/families and staff.
  • Performs and documents weekly review with Medicare Review Consultants per facility practice.
  • Evaluates data to revise the plan of care to reflect resident current status and meet the changing needs of the resident.
  • Evaluates response to interventions against expected norms for age and development, resident's individual strengths and preferences.
  • Identifies and documents resident/family learning needs and provides education based on assessment in collaboration and coordination with resident, family, and interdisciplinary team members.
  • Documents resident/family response to education.
  • Coordinates with IDT and resident, need for and eligibility for continuation of skilled care, evaluation of need for change of level of care and/or discharge potential.
  • Maintains working knowledge of CMS Regulations so that documentation supports and reflects compliance.
  • Maintains working knowledge of Medicare skilled and MDS criteria so that documentation supports the need for care, as well as, reflects resident status and is completed within the allowable timeframes.
  • Incorporates resident needs and preferences into organized, efficient plans for delivery care by a team assigned for daily cares.
  • Collaborates with IDT to evaluate facility QI's generated by the MDS transmissions, ECU generated
  • Quality audits and other external source data to assure that coding and facility practices provide for quality care and works thru process improvement to remedy areas of deficiency.
  • Ensures implementation, continuation and evaluation of Restorative programs.

Required Qualifications:

  • Graduate of accredited school of nursing
  • RN licens
